Can anyone advise how to fine tune basal please for Toujeo.. I've been on Toujeo for a week thought it kept levels steady for 24 hours. My morning blood sugar was 11.4 this morning I have increased Toujeo by 1 unit now at 23 injected at midday. My problem is mainly after breakfast where level shoots upto 17 and feel quite unwell? It was behaving for first few days and I've not done anything different. I inject 2.3 units for every ten carbs using novorapid.

Before pump I was on 21 units of Lantus at breakfast then 1 unit to cover 10 carbs, 1 unit used as correction to reduce by 3 although insulin sensitivity decided to kick in.

I was on a pump have come off now a very long story.

Thanks

Hi @Tracie1212 here's a link for basal testing https://mysugr.com/basal-rate-testing/

As with any change of insulin it will take tweaking and adjusting for a good few weeks to settle in so as hard as it is stay patient and don't panic if you don't see instant results, we can't advise on changing doses but the basal testing will at least identify what's going on, good luck.

Hi@juicyj

Thanks for link.

Doctor told me to change Toujeo by 1 unit if higher in morning which I was so did change at midday today. Now I'm going low going to be a long night no basal testing as trying to keep levels from dropping too much. I've had enough!!

Are you sticking to the 3hr window of what time you should take your toujeo? Toujeo lasts longer than lantus, by a far bit so no split doses are needed.
Mine is Toujeo300, is yours?

Hi@ickihun

Thanks for your reply.

I've not been advised about 3 hour window can you elaborate please. I was told to take at midday everyday which I have done for past week. For some reason run high after breakfast. Yes mine is Toujeo 300 too. Do you take yours at same time everyday? Which fast acting insulin do you take

Thanks

My fast acting is novarapid. Any leaflet about Toujeo300 will tell you that must be near same time of day, everyday, but not to exceed 3hrs over.
Ive tried other times of day but on waking suits me best. One job done then forgotten about. My need has doubled since a few infections.
Just be careful with fast acting when increasing basal. You may need less fast acting.?

You are probably experiencing dawn/"feet on the floor" phenomenon. Many of us require much more insulin in the morning. I take 1 unit of NovoRapid the second I get out of bed, whether or not I eat, or I will skyrocket within an hour. Even if Toujeo works exactly as you expect it to, a 24-hour flat basal, this can't counteract the dawn phenomenon as your liver is releasing its glycogen stores, causing you to need more insulin. Pumps can be programmed to deal with this.

Basal checking is highly recommended thou.
Ive increased basal with no effect, if anything I think my liver just dumped all the more glucose.
Dropped my basal back down and now seeing less resistance. However I'm type2 so I don't recommend dropping basal if nurse hasn't suggested with you being a type1.
