Blood Sugar Rising Between Meals

Hi All,

I was diagnosed a few months ago. Since then I've mainly been eating each meal around four hours after the last. When checking two hours after eating, my blood sugar rise is within normal range. It's still normal before the next meal.

I've noticed that when I leave it five to six hours between meals (after the NovoRapid has finished), my blood sugar will rise substantially before the next meal. From say 5.0 to 9.0.

Is this a basal issue? I take Lantus at 10pm each night. Overnight my blood sugar will hardly change, It's only during the day I see such a rise.

It sounds like your Lantus is running out of steam the next day. It is not unusual. Many split their Lantus between evening and morning shots to get full 24 hour coverage.

Could switching to once daily Tresiba be an alternative to injecting Lantus twice a day? Because of the longer acting profile.

Lantus in my experience is a funny 'ol basal. Novorapid for me can have a little lick in it's tail about the fith hour?
In your position, I would look at basal testing to find out if it needs a tweak first?
Though from the sounds of it, you ain't doin too bad..

Tresiba once a day may work well for you, especially as you are recently diagnosed and still making much of your own insulin. People with no residual insulin production struggle with very long acting insulin because it doesn't cater for the typical increased morning insulin requirement. This can be done with a twice daily dosage split using Lantus or Levemir. But your pancreas may fill the gaps with Tresiba at this stage. Discuss it with your doctor and try it out. If it doesn't work, split dose with a shorter acting basal would be the next thing to try.
