Confused about High BG in the morning

Hi everyone,

Since changing from the Levemir onto Porcine I have felt so much better and have everything under control except at night.

At 11pm I take my Porcine 16 units, and depending on my BG I will take a correction dose of Humalog, the last two weeks I have woken at 4am and checked BG which is usually around 5 - 6 so I am happy with that, but when I check again at around 9am its been around 17 - 20 :?

So I tried injecting another 6 units of humalog at 4am to see what would happen and still at 9am its back at 17 - 20. So I tried increasing the Porcine to 20 but still same results.

Now I am confused as what to do, I do not want to increase the humalog for fear of a hypo.

Any ideas? what is odd is in the morning I can take 8 of humalog have a banana and my BG is "normal" by lunch time. :? :? :?

Something strange is going on. :lol:

Hello Fugi

I think you ought to make appt to see yr dsn or consultant and explain to them what is going on. From reading yr msg, it sounds like you are experiencing DP which is common from 4am onwards. I also get it as well sometimes.

Have you thought about asking for insulin pump therapy as that will enable you to alter the basal rate of insulin. I had a go using a pump just recently and was quite taken with it. It sorted DP for me. I am now back using MDI again just to see what the difference is in control but for sure I will be either buying a pump or getting one through my PCT.

Pumps are brill :) I took to the one I was using like a duck ............ :D

Not too keen on a pump not sure why, its the thought of having something strapped to me. Probably not rational.

Now I have to ask what is a DP :oops:

Will now read up on it, only had this since changing insulin, and because I feel so much better on porcine I am not keen to change.
 
Not really sure what to say here Fugi. Could be that the porcine insulin for some reason is not agreeing with you as you should be seeing a lowering of bg with the extra Hlog and upping the porcine as well. Might be as well that the Hlog refuses to work ok with the porcine :?:

I doubt if you are going hypo after 4am as you would wake up yr wife sweating buckets although its a possibility. If you are, then yr liver might decide to come to yr rescue and release some glucose thus sending you towards being high. What is yr bg level at 7.30am?

There are a few threads on this forum about DP.

DAWN PHENOMENON
Everyone, diabetic or not, exhibits some Dawn Phenomenon. It is a natural part of our bodies' circadian rhythms. Some have said it is the way our ancestors had the strength to rise and slay a wooly behemoth for breakfast.

Since most of us fast while sleeping, with teenagers a possible exception, our bodies use stored energy during sleep. The body uses all three macro-nutrients (carbohydrates, proteins, and fats) to store energy.

The most easily used is the storage medium of carbohydrates, called glycogen. Glycogen is made from glucose, and is stored in the liver and muscles. Since it is basically nothing more than a complex matrix of glucose, it is easy for the body to store and use, something the body does all day long. The technical term for the act of creating and storing glycogen is glycogenesis. When the body calls for the conversion of glycogen back to glucose it is called glycogenolysis.

Another macro-nutrient that is available to be converted to glucose is protein. Most of us think of our protein as being stored in muscle, but the body has protective mechanisms to make muscle wasting its last choice. One of the most useful and readily available sources of protein storage is in blood components, i.e., albumin (plasma). The body uses a process performed in the liver to convert amino acids, the building blocks of proteins, into glucose. The name for this process is gluconeogenesis, literally "the creation of new glucose".

So, what does all this have to do with a high fasting BG? Overnight, usually between 4am and 11am, your body releases some hormones. These are Growth Hormone (GH) from the anterior pituitary gland, cortisol from the adrenal cortex, glucagon from your pancreatic alpha-cells, and epinephrine (adrenalin). These hormones cause an increase in insulin resistance, raising your BG. In addition, these hormones trigger glycogenolysis and gluconeogenesis, adding stored or new glucose to your bloodstream.

Thanks for the replies, just when you thought you have your head round it all, something complicates matters. :lol:

During the day I have the control pretty much as I want it. Health wise feel 500% better than I did on Lantus and 100% better than on Levemir.

I suppose the good thing is even when high in the morning I can bring it under control really quick which I could never do on the other stuff.

Still its all sent to try us. :mrgreen:
