If on a pump and your basal testing is ok, then there are basically two choices of what may be wrong.......you say you have always had the same carb ratio.......I would suggest that this needs adjusting.....also, depending in the time that you have programmed in to your pump for insulin working time, this may well need slightly adjusting.
If you're going high/low within 3 hours of eating then you need to adjust your carb ratio either up or down.
If you eat two meals too closely together (as I was doing with my breakfast and lunch initially) then this may too have a contribution towards levels going higher. I was eating at 9am and again at 12noon-only 3 hours apart for my bolus's, so this was affecting me, as my insulin working time was set to 4 hours I think....now I always try to have my main meals 5 hours apart, and my insulin working time is set to 5 hours too. This helped a lot.
Please don't feel fed up, it just needs working through. I had a hysterectomy 15 years ago and am currently 47, and so far (touchwood) I can't say that hormones are affecting me, however your GP could do a test easily to see what's happening with your hormones.
Personally, I ate a lot more during Christmas this year, and despite eating less for a month, it is taking time for my body to adjust back to the less food, and I have had to alter basals and bolus rates.
It is a blip only, if you have been okay for 3 years you will pull it back, no doubts about it. There are a number of things that could be affecting you, so I would ask GP to see if he/she could test hormone levels, but at the same time I would start off by looking at the timings of what you are eating, and your bolus ratio which may need adjusting.
