Hi,

I am on 90mg of Gliclazide MR daily having been diagnosed with T2 diabetes about 6 years ago. I have had my dose upped recently from 60mg after having some high HbA1c results. Unfortunately, upping the dose doesn't seem to be making that much difference and my spot blood sugar counts are still high. I have been on 90mg now for nearly 6 months. I am not overweight and exercise daily. I am wondering in Gliclazide has lost it's efficacy.

Has anybody had a similar experience?

If the Glic has lost its efficacy,and you don't have insulin resistance it may be that your pancreas beta cells may not be able to produce the insulin, so testing for T1.5 and a move to insulin may be required. Ask your doc for a c-peptide& antibody test.

90mg Gliclazide a day is not a particularly high dose, and if its not doing the trick you should see your doctor to discuss increasing it. (Until recently I was on 160mg Glic twice a day). It works by forcing the pancreas to produce more insulin; unfortunately this solution doesn't last for ever as after a while the poor old pancreas begins to "wear out", and you either have to increase the Glic, or try another tack. (I'm now on Byetta, and the Glic has been reduced to 80mg twice a day.)
Everyone is different, so you need to dicuss with your doctor a strategy for dealing with your particular set of circumstances; the solution you come up with may be quite different from mine!

Marvin, as i recall this is Glic MR. the little 30mg tabs equate to 80mg non MR so the OP's 90mg/day equates to a normal 240mg/day.

Thank you for your replies. I am not sure that MR equates to a higher dose so I guess I have a way to go on Gliclazide. May GP has suggested previously that moving to an insulin as well as tablets sooner rather than later will offer better control - any advice?

Hi. It does sound like your islet cells are failing. I was on 320 mg/day of standard Glic and it had little effect. As you are not overweight I agree you could be T1.5 so ask for the two tests to be done
