Help please about diagnosis for my 2yr old

Hi there,

A week ago Friday, I took my son to the doctors with diarrhoea that had lasted 2 weeks, high temp and complaining of tummy ache,doc thought something viral but picked up on the fact my son hasn't gained weight for 7 months, she sent him for blood test, long story short we were called in and asked about his drinking and toilet habits, my son does drink alot hence he goes to the toilet alot, she then said due to his tests we need to send you to the hospital as we think your son has diabetes, at the hospital his urine was tested (no ketones) and prick test done and doc said that was fine, so discharged and went home. Next day my doc phones to ask how we got on, I told her they said he's fine and she said she was upset with that and told me she had done a hba1c that came back at 48, and that I was to wait on another appointment to see the doctor again for further testing, so I guess now its a whole worry of has he hasn't he, is it possible to have diabetes and it not show ketones in his urine? Or is there another reason why my sons hba1c is high? I like to think he has a very well balanced diet I wouldn't say he has too much sugar as my husband is a Heath freak so we hardly have take out or pick out on sweets. Any help on this matter would be greatly appreciated.

Hi. An Hba1C of 48 is slightly elevated but not bad; above 58 is considered bad. It's possible that there could be another cause in the background that has resulted in the slightly elevated blood sugar. Ketones would normally only be present when the body is producing very little insulin and blood sugar, HBa1C, is perhaps, above 140mmol. The GP is right to continue to follow-up. You could get hold of a glucose meter and check your Son's blood sugar 2 hours after a typical meal. It should be below 8.5'ish to be considered OK. I think you need to wait for further tests but I wouldn't panic over a result of 48.
 
Hi my dear. Please don't worry yourself. It looks like you have a very caring doctor who just wants to make sure everything is OK and nothing gets missed. More tests is a good idea. Thinking diabetes is related to diet & sugar is the norm, there are actually 2 types of diabetes. Type 1 is autoimmune and has nothing to do with diet. The pancreas begins not to produce insulin anymore. This is the type kids can get - cause unknown and not anyone's fault. Type 2 is the more common form that can be diet related. 48 is higher than expected but not as high as some that present on diagnosis. You can have high glucose and no ketones. No ketones is good, but presence of glucose suggests something is amiss. They can also test the blood to see if any autoantibodies are present.
