Hopeless annual diabetic review

Firstly the headline figure HbA1c of 37mmol/mol or 6.2mmol/L / 5.5% in old units (Previous years 2012 back to 2009 have been 40 42 42 37)

I 'failed' a self administered OGTT just three weeks ago 4.9 mmol/L waking, 14.5 @ 1 hour 12.6 @ 2 hours (with finger tip testing) Clearly impared glucose at the very least and and over the limit for diabetic diagnosis although I admit there is some error introduced by the use of the finger tip test

Figures on waking from the last three months range from 3.9 to 13.5, but mainly in the mid 5's to late 6's, very blurry eyes on waking is a regular occurance, numb feet and legs quite often

New nurse at GP practice really chessed me off with her attitude. No blood pressure check, no weight check, no test of neuropathy, no real indication if my figures for other things were ok. Refused testing strips as 'not diabetic' despite years of usage to ensure safety when driving and operating machinery, told if I am to be declared diabetic, I have to have an OGTT administered at their premises and pay a prescription charge for 'Polycal'

I had a huge number of blood tests in 2001 when I had very severe fatigue and chest pains and among them was a fasting blood glucose of 8.2. Despite this the 'experts' at the local hospital and the GP failed to diagnose me, the physician eventually said I was 'unfit' No **** sherlock I could hardly bloody move let alone walk or run or cycle! Confirmation of diabetes with a self administered OGTT of around 13 @ 2 hours was accepted by the doctor eight years later in 2009 and was scanned and is in my records at the surgery, I've had a few years of retinal screening with no issues. But there is lots of family history of diabetes and pancreatic problems. Presumably with this new policy its now deemed cheaper in the short term to drive a patient away and maybe off their books than actually provide proper preventative assistance by the way of regular testing?

Managed to get a printout of my figures although she was very reluctant to give them to me

Other results:

Urine albumin 7.5mg/L
Urine creatine 13.6 mmol/L
Urine albubin:creatine ratio 0.6mg/mmol

Serum cholesterol 4.7mmol/L
Serum HDL 1.4mmol/L
Serum triglicerides 1.1 mmol/L
Total cholesterol:HDL ratio 3.4
Serum LDL cholesterol level 2.8 mmol/L

Serum sodium 142 mmol/L
Serum potassium 4.2 mmol/L
Serum creatine 84 umol/L
Serum urea level 3.2 mmol/L
GFR calculated abbreviated MDRD 84ml/min/1.73m2

Forgot to add that back in 2011 they wanted to put me on Metformin !!

Why are you getting those bloods done if you are not a diabetic?

I'm not sure what you are asking for in your post though? For a diabetic your HbA1c is good.

Why don't you just get the OGTT done and clarify the issue; then if you are you'll get better support.

Yes the HbA1c is good but waking bloods of 13.5 is not by any measure 'normal' The checks and support has been there in previous years, this year it just evaporated with a change of staff. The diabetic check this year was about as good as someone with a reading age of about 5 could have managed.

Obviously you felt your nurse wasn't listening or explaining things to you which isn't good but the blood test results she has in front of her are fine.
I would have thought that properly conducted oral glucose tolerance test would be a good next step.
Sadly most T2 diabetics who are not on hypo inducing medications are unable to get strips either in the UK (and to be honest much of the rest of Europe and elsewhere in the world)

Your Tests:
HbA1c is normal http://www.diabetes.org.uk/About_us...vention/New_diagnostic_criteria_for_diabetes/
urine albumin:creatinine ratio : is well below 2.5mg/mmol if you are male or 3.5mg/mmol for a female and shows that you don't have microalbuminuria ie the kidneys are effective and you aren't excreting protein in your urine http://www.patient.co.uk/doctor/Microalbuminuria.htm
Cholesterol levels are all under 'targets' http://www.patient.co.uk/health/cholesterol
eGFR : 'People with an eGFR of 60-89 without any known kidney damage or disease are not considered to have chronic kidney disease (CKD)'

That is a sad twist of things - work hard to get your diabetes under control & you're told you are not diabetic & lose all the support that has helped you get control. Ask around at local pharmacies to see which Drs (not witch doctors) are likely to be more positive.

I'll go for the OGTT as I don't really have any choice. If I had gone on Metformin as they wanted me to then I would probably be judged a success of their treatment regime. Despite relating occurences in the past year of waking blurred vision, and headaches with high BG, and irregular working / driving patterns together with operating potentially dangerous machinery I was pushed out the door and presumably a box was ticked to satisfy the powers that be. Can't help thinking that I'll get the same respose at all the local practices, that's if they have space, my current one appears to be the only one in the area advertising for new patients and presumably only very healthy ones at that.
