Yes might sound odd, but I was a type 2 diabetic reasonably controlled with metformin and gliclazide for 15 years, then I got a blocked bile duct and had a routine procedure that ended up with me being in intensive care with sepsis, septic shock and acute pancreatitis. And after two further emergency trips to A&E, a week or so after leaving hospital, with acidosis , was declared an insulin dependent diabetic! Never got to see an endo, and eventually got a nurse to introduce me to insulin ( 24 hour acting humulin).. about 10 units in bum or tum before bed. My Hba1c has been raised recently over lockdown ( I do have other medical conditions with hypothyroidism and fibromyalgia, and being tested for sleep apnoea as developed resistant hypertension too...which might have affected it), and on the phone nurse now says inject insulin before bed and in the day. She was rather vague about this, and I couldn’t get an answer as why I needed ‘a top up’ of a 24- hour insulin. I tend to do the second after lunch. Fasting bloods tend to be about 4, but this morning 3.4, so I don’t need the second dose 12 hours after the first. Anybody any advice? Can’t have another Hba1c test for another couple of months. Incidentally still on gliclazide and metformin...nobody suggested changing their dose or use since taking insulin in the last 6 years...any thoughts?
