I'm no expert, but this is what I do...
Test before meals. Calculate adjustment dose if high and add this to the carb counted food dose. Record time, BG, carb intake, adjustment dose, total insulin and brief meal description.
Also test and record bedtime BG. As I take Lantus at night, this helps determine that the background dose is about right.
Any hypos are recorded along with the amount of glucose consumed.
It's well worth the hassle in terms of feeling under control.
