Hyper Advice Please

Hi, just looking for a bit of advice regarding my 14 year old nephew's type 1. Since he came to stay with us, we've been working hard with the DNS's to get his blood levels to a better level; prevoiusly over 20 most days. He takes Lantus at night and novorapid 4 times per day.

Over the past 3 months we've gone from high, to hypo to stable (ish) but over the last 4 days it's back up to the mid teens with no aparant reason. I don't want to keep pestering the DNS's all the time as I'm phoning every week.

We've checked keytones, which are fine. We're changing injection sites routinely and washing hands etc before testing.

Should I leave this for a few more days to see if it settles or phone up the DNS?

Without knowing more its hard to give advice but its very likely he needs to learn some dose adjustment......

The first dose to get right is the Lantus.....then the novorapid....

Does he carb count currently...?
 
hi, yes he does carb count. He has the aviva expert machine which works out his bolus by differing ratio's for each time of the day. My concern is it's gone from being under control to very high, very suddenly.
 
Could he be having extra food without you knowing? Is he ill at all? Or under any recent stress?

Have you changed both his I sulins to new cartridges/pens in case one of them has become ineffective? That's the first thing I'd try.

Hi Azure,
He's assured me that hes not secretly eating and he's not ill (done the keytones) He is under a fair bit of stress at the minute but I thought it might've settled a bit over the last few days.
I'll try new insulins today to see how that goes.

Stress can have a surprising effect on blood sugars so that could well be a possible,cause. But it's definitely a good idea to change the insulins anyway so you're confident that's ok.

If the highs persist, then it might be worth a call to his DSN. I know you don't want to bother them but it could be that they might advise a slight increase in insulin to,cover the stressful,period, or just correction doses. For myself, if it's shortlived stress, I usually just correct, but that's just me.
 
might be worth testing the Lantus dose if he has his ratios already established.....

just to be sure....

stress is a contributing factor right enough.....

insulin requirements change all the time, sometimes more regularly than others....so if it keeps up just go through the motions of testing each dose...

How long has he been diagnosed? He may just be starting to come out of the honeymoon period, where levels increase and ratios will need to be changed. I am 17 and have been diagnosed for 6 months now, and haven't ended mine yet. When i was first diagnosed, i was ringing my DSN every day, or every 2 days for the first 2 months. Now i ring every few weeks and see them every 2 months. Don't be afraid to ring them more!
