Simply put - if trying for a baby and there is unexplained male infertility a key test is to get fasting insulin done, if that figure is more than about 8 MiU/ml ( the lower the better) then that person has a degree of insulin resistance which can be calculated using HOMA . It will also show up as higher than average triglycerides . Studies show that that makes it harder to impregnate.
It seems that this type of male infertility might be the male equivalent of PCOS.
