You could try adding wheat gluten to the usual options for low carb baking. It is the gluten which holds the CO2 in from the yeast and makes the bread rise.

Try Vital Wheat Gluten and see if it spikes your sugars. As long as you are not allergic to gluten this is also 4gm of carbs per 100gms.
