Ketoacidosis

.... I don't understand now is why insulin reduces ketones so I'm off to read about that. ...
Lots of ketones and ketoacidosis are caused by lack of insulin. The body starts converting protein and fat to energy and ketones are a by-product of the process. So when insulin is supplied ketones drop. Have you been losing weight?

It sounds like you could be LADA. An antibody test will give the answer. In the meantime, continuing with the insulin is probably good idea.
