Ketosis Prone Type 2

Hello, at the beginning of Feb I went into DKA. I only found out I was diabetic a day before that. I took 2 tablets of Metformin and a day after I collapsed. I am on insulin now (4 injections a day). My GAD and IA2 tests were negative. My c-peptide were within norm. I was told I might be Ketosis Prone Type 2 diabetic (I am type 2 but when my sugar is above 14 I need to start measuring ketones in my blood). Has anyone got any experience with KPT2? Diabetic nurse specialist told me that my pancreas produces enough insulin and the plan is to wean me off insulin and try antagonist GLP-1 instead. Any thoughts on that too?

Welcome to the forum if this is your first visit post diagnosis.
Firstly it is good that your c peptide is normal. If you can reverse your insulin resistance (cells becoming deaf to the insulin signal when the body tries to deliver energy (glucose) to the cell) then I'd hope you would be able to come off insulin. You've been offered meds that help your body produce insulin in a more effective way but diet is likely to be the most effective way to reduce your need for meds so its worth looking here for ideas. Many people suggest a low carb approach but there are various different ways to get started on improving our diet.
Ketosis to a dangerous level is rare for a type 2. I am type 1 and it can happen if my blood glucose rises to pathological levels (above 6) because my body can't use the energy I am eating and as alternative fuel is making its own energy from my liver - ketones.
At a high level these ketones are toxic and lead to ketoacidosis which can be fatal. You can be in nutritional ketoses if you eat much less carbohydrate but this isn't pathological. and is actually beneficial if you are trying to reverse type 2 diabetes.
You might have had high blood sugars and have had higher than normal ketones without this being dangerous i.e. ketoacidosis. Normal symptoms of ketoacidosis are pretty extreme e.g. shortness of breath, extreme vomiting and urination, excessive thirst but can't keep fluids down.
Have you been given test strips for this. You can get blood ones or more likely, urine testing strips.

@NicoleC1971 thank you for your reply. I think I am giving doctors a headache why I collapsed being type 2. I had covid at the beginning of the year too..
I will be under consultant care for now. My diabetic nurse specialist said that type 2 usually goes under GP care but she is going to reffer me to see consultant for him to make final diagnosis and decide about the treatment.
Yes I was given second monitor to take home. It measures ketones and I was prescribed ketones strips too.

It is all confusing.

I am vegetarian so I look at what I eat. I am already on low carbs diet. I was refferd to see dietetician but I haven't heard anything yet.

If you are truly eating low carb yet still getting blood sugar measurements of 14 mmol/l then I'm afraid you either need to cut carbs even more dramatically or something else is wrong.

Would you like to share some normal meals with us so we can see how "low carb" your diet really is.

I'm assuming you are vegetarian for moral reasons rather than for your health?

If you're insulin production is normal but your blood sugar levels are high then your diet has to be the most likely culprit. You are more than likely consuming more carbs than you realize and more than your body can handle. It's not impossible to eat low carb as a vegetarian but it can be more of a challenge. There is a dedicated section for vegetarians, you could possibly get some answers there

Thank you for kind suggestions. Since I was discharged from hospital I only had once level of 14 at home. It was in a first week back home. My levels are between 7 and 9 now.
Writing about sugar level 14 I simply was referring to the advice I was given by the nurse.

My main wonder is whether anyone else was diagnosed to be Ketosis Prone Type 2?

A bit of my background for you:

I had blood tests done in November and other than low iron level it was ok. I was told to repeat them after 6 weeks. At the beginning of January I had covid. After I came out of isolation I went to have my blood tests done.
I got a call from my gp on the 7th of Feb and was told that I was diabetic and I was put on Metformin twice a day. On the 8th of Feb I collapsed, had feet and hands numb and was taken into the hospital. I went into DKA, had ketones in my blood and I was on a drip. They flashed 8 bags of fluid through my system and after a week I was discharged.
I am on insulin now. I take 4 injections a day plus tablets twice a day.

Hi. The C-peptide test is not the most reliable test and it's worth having both a plasma (bloods) one and the urine one. I'm in a similar position and my consultant says he will treat me as T1 despite my urine C-Peptide being above T1 level. When I had a private plasma one done a few years back it was much nearer to the T1 level.
