Lantus Starting Dose

hi guys,

just wanna ask is it normal for someone with diabtes type 1 for 18 years to change to lantus and take only 10 unit.?

let me explain: if someone with 150cm and 42kg was taking Insulatard and Actrapid for 18 years, then changed to lantus and found out that 10 unit of lantus is working for him and 7 unit of novorapid sipilts into 3 for morining, 2 for launch and 2 for dinner. he was taking 25 unit of insulatard and 15unit of actrapid.

is lantus is that stong when we compare 25 of insultard = 10 of lantus?

I read from lantus offical website the lantus must be 1/3 of the total insulin, or must be 0.4 or 0.8 of body wegit in kg.. is there a rules or what?

should this person increase his food or insulin?

and the most important thing, is this person consider as a good healthy diabetic because some with 18 years with diabetes would need more insulin than this?

Lantus should cover 30% to 50% of total daily needs. Those are general guidelines not strict rules. Starting dose is 0.2 u/kg. I was on 32~ units of insulatard and ended up on 19 units of lantus so the figures are not strict. If 10 units is working then that is great!! No need to change anything. To confirm that 10 units is enough, you only have 1 step to do: basal testing.

The only rule is: what works for you works for you,
Everyone is different, so what works for one may not work at all for another, even if they are the same height and weight

Basically you fast for 4 to 12 hours after eating, you can't sleep, sugar must be between 100-200 mg/dl, no hypos within the last 6 hours, no exercise. Just continue your day normally and test every hour. The sugar should not move more than 40 points up or down. If it goes up, Lantus is not enough. If it goes down, Lantus is too much.
