Levels after food

Two hours after meals my levels are never under 9 if I kept them that tight would be in hypo land. What should they be after 4 hours? Theory and Practice. I don't have huge amounts of carbs tried nuts for snacks but don't work for me. Can't eat eggs so always trying to find another low carb snack I eat cheese everyday. Starting to get fed up of baked beans. Eat lots of fish Dietician told me to eat the skin of baked potato slows down the release.

What's your bg levels before eating, what do you typically eat in a meal and what are your levels 4 hours later and are you snacking in between ?

I aim to be around 5.5-7 mmol/l 4 hours after eating as a target without eating any snacks in the meantime, if I've got this right then I've done a good job with my bolus/carb count. If I eat snacks then I bolus accordingly so I'd still do a bolus for eggs as the protein will convert to glucose.

My morning level normally 5.9 I only have one shredded wheat and 1 slice of multi grain bread. Cup of tea.

Im on mdis luck I have a Libre so that helps with monitoring.


Total 34 carbs breakfast every morning.
Injected 6.5 units. This varies
2 hours later levels 11. 9.

Had a busy morning by 1.30pm levels at 13.6
20 carbs for lunch
Injected 5.5 units
3pm very tired ate oat biscuit 12carbs felt unwell after my level 9.7. injected half dose continued to feel unwell.
5.30pm levels 6.1

All afternoon levels around 6/7
7pm 40 carbs 4 units
11pm toujeo 22 units always feel unwell after injection plus levels drop by 3.
Level 13 before injection
Now fallen to 9.

Have to eat biscuit to get me through night without hypo.

If I snack usually leaves me feeling unwell so try to avoid.

Ive been monitoring my levels with novorapid, even with prebolusing I can see large spikes if I have a sandwich. What I have noticed is that it does reach its peak ~ 2hours after injecting, but my levels are still around 10/11 at this point, by 3 hours they drop to around 8/9 but the last 2 hours it can drop another 5/6 units leading to a hypo. This 3 hours of staying high even with pre-bolusing is one of the main reasons I will be asking to move to FIASP on my next visit with the diabetes specialist. The action time and length of action is not ideal, neither is FIASP but its the better of the two evil's.
 
Sounds to me like you are rising in the morning which has a knock on affect for the day. I agree that you may need to pre bolus especially for breakfast. Use your Libre to see how long before things start to move downwards. Also consider less carbs in the morning so the morning rise isn’t exacerbated. Perhaps some berries and Yoghurt.
For me I personally would correct after 2 hours. I wouldn’t have done this pre Libre but using the trend arrows now feel confident to do so. Obviously wouldn’t on a downwards arrow.
The other equation is basal testing. Boring but essential to see how that Toujeo is doing. I too am on Toujeo by the way. Not many of us.
 
Have you had a look at the basal at these times......maybe it needs adjusting.......

I'm using novorapid my food peak is 90 minutes have discussed with DSNs who advised to inject straight after food.

Think our experiences are very similar@muneeb.

I discussed Fiasp with my DSNs she thought may be too fast acting for me.

I'm up most nights sat on sofa monitoring my levels as go low early hours on Toujeo. If I reduce background dose don't feel well and my Ketones rise.

How long have you been on Toujeo @becca59? How are you finding it. I take 22 at 11pm. Levels drop after injecting and get palpitations Do you have same after injecting?

Have you considered a pump? Sounds to me that your basal is too high at night but too low in the day so maybe a pump is the way to go????
But more basal testing may be the way to go. Have you done any night time checks to see if your libre ia accurate then (hint, it may under read).
 
Hi, yes tried omnipod pump didn't work out for me it's a long story

Libre readings vary usually by couple of levels. I take blood tests for meal times and checking highs and lows.

Can I also ask which insulin you were using before Toujeo I am about to start with Tresiba but very worried about the overlap and going hypo. DSNs say use same doses of 22 per night.

I've named my hypos jumanji that's exactly how lows make me feel. Heart beating like a drum ....

Hi @Tracie1212 sorry for delay in replying. Was changed to Toujeo when on DAFNE nearly three years ago. Previously on Lantus. I too was having highs in the morning, lows at night. I changed timing of injections to the mornings and that has helped. Am only on 9 units at the moment. Took 11 in the heat of the summer but needed to reduce as the weather changed.
I haven’t noticed any reaction when I inject it. Good luck with the change.
 
Novorapid has a very long profile with me. No point in checking after 2 hours because if I correct then I'll hypo in another hour.

On a day with barely any carbs so just relying on background tresiba my levels stayed at around 6 - so I know the problems are food vs novorapid vs exercise.
 
Tracie your levels sound very similar to mine especially my morning BG

I will wake with a BG of say 7, take my 40g of carbs, porridge,. About 2 hours later my BG will have shot up typically by 4--5 points, so BG of 12. After 4/5 hours though, just before lunch it will have dropped back to 7, where i would expect it to be.

I am also using Novorapid ( or Novo sluggish as i call it)
