Libre 2 Plus availability plus cracked Novorapid Penfill cartridges

A couple of current problems. First my pharmacy cannot currently get hold of any Libre 2 Plus sensors but can get Libre 2. Has anyone else hit this problem? Secondly a few months back a few Novorapid cartridges cracked in use in the NovoPen 6. I reported this to the pharmacist and after bit of pressure I was given a new pack. All was well for a couple of months and then this week I had another crack. I've reverted to my older NovoPen 4 in case the newer pen was applying too much pressure etc. I reported the failure to NovoNordisk and I will be returning the faulty cartridge to them. My fridge temperature is not too low causing freezing. A year ago Novo had faulty NovoRapid cartridges and issued a public warning although the person I spoke to at Novo was unaware of that (!). Anyone else come across the problem?

Apart from a lot of memory errors, my Novopen plus isn't giving any other issues. No problem getting the Libre2 plus either

Yes only had the 1 though and just threw it away

I have had a problem with one of my Tresiba cartridges, it started leaking every time i used it and on inspection i found a crack running down the full length of the vile. obviously causing problems as i am now unsure how much insulin i have been getting, i only noticed it by chance as the cold liquid ran down my belly lol

I was getting high blood sugars and couldn't understand why !! So did a test of 22 units into the needle cap and hardly any went in. Problem solved

I had a day of hypers early December, until I spotted a cracked NovoMix 30 cartridge.
Then last week, more hypers, no crack, but the cartridge was clearly not injecting. I noticed that there was a "blowback" of some blood into the suspension. Never seen that before.
I informed the Pharmacist and my surgery. The Pharmacist sent a photo of the cartridge and a report, to her yellow-card site.
This seems to be a quality control issue, which has been evident for some time. If so, it's about time Novo Insulins sorted it out.
It means I cannot rely on my Insulin system when I travel away from home, unless I take some spare cartridges with me.

Another out-of-control day yesterday. The NovoPen cartridge was very reluctant to "pressurise", there was the smell of the preservative plus some obvious leakage.
Close inspection showed the stub of a previous snapped off portion of needle, in the bung. This is what was causing the pressure failure and leakage.
I now inspect the bung plus the used needle to ensure that this doesn't happen again.
Has anyone else noted this problem? Could it be that there are quality issues with the needles, as well as the occasional split cartridge?
I've informed my Pharmacist, who's not yet replied.

Following a call to Novonordisk I returned a faulty cartridge to them and asked to be copied on their report. As expected they found no problem with the manufacture of the cartridge so side-stepped the problem. It never happens does it!
