Maximum Lantus dose.

I cannot find any info regarding a maximum dose for Lantus online.

Two week ago a switch got flipped in my body and my sugars and ketones went out of control. Appointment to see the doc is not for another 2 weeks. and some blood sugars neared 400mg/dl.

Ketones came under control once I stopped my Farxiga, but I am trying to compensate in the interim with additional Lantus.

My old dose was 110 u/day. I am up to 200u/day currently and my fasting numbers are still >200mg/dl, with <10g carbs for the last 2 days.

Something weird is going on. Anyone taking more Lantus or know what a maximum dose is. I"ll worry about highs and spikes with the doc, but I don't want to be sitting at 300mg/dl for 2 weeks.

Hmm, according to this article, i still have a ways to go.

Getting high blood sugar levels under control

A 70-year-old man with a history of type 2 diabetes takes insulin glargine (Lantus) 100 units every morning and insulin aspart (Novolog) 20 units with each meal. However his blood sugar is still around 250 mg/dL.

How are you managing to inject such a large quantity of lantus? I know some insulins have U500 versions (5 times the strength) so as to reduce the volume needed to be injected, but can't see a U500 lantus online.

Are you taking a fast acting insulin as well?

If I've understood your previous posts correctly you are still producing large quantities of your own insulin so insulin resistance is your issue? (Though I suppose the increase could also be explained by a sudden reduction in your own insulin production or some combination of the two factors).

Currently in 3 injections a day of 66u. Tomorrow will be 68u each.

I am on no fast acting insulins.

Correct. My C-peptide this week came back still above normal, so still indicating I am producing a good bit of endogenous insulin, (We tested it again as we also though that perhaps the pancreas had just given up the ghost to explain the change.)

I have a theory (but no medical evidence or background to back it up). I have mitochondrial disease which means when the glucose enters my cell it is not utilized very well. I think the sudden "insulin resistance" may just be a "glucose utilization" problem, and you can only shove so much sugar inot the cells before they are saturated.

In any event, I only have two tools at this point. Insulin and starvation.

This could be way off the mark I know, but have you tried splitting each dose of lantus into 2 injections taken at the same time, but well apart from each other? It 'could' absorb better that way.

The pen has a maximum "plunge" of 80 units. I do 3 injections daily of (now 68u).

I could give it a shot. I do this occasionally when one pen runs out and I have to get the remaining dose from a new pen. I haven't noticed a difference on those days.

Rather than starvation, do you reduce your carb intake but increases proteins and fats
This is a common approach from many of the people of the forum with insulin resistance - they reduce their need for insulin.

I am limited to 10g of fat a day. That only leaves protein and that does turn into sugar eventually. Indeed, for the last 3 days, I haven't taken in ANY of the things normally associated with carbs. There ARE some carbs in cheese and eggs, but that is all.
