That is exactly what I was.
Now i don't smoke, don't drink, exercise daily, eat a really healthy diet and have my diabetes under control and have lost 35kgs. Still having a great time.
Worse thing is I never saw a fat man in the mirror. I saw a large guy but not fat. I wish he had insulted me I would maybe have responded to that direct approach.
I am no longer a fat lazy porker who's only exercise was walking to a bar or to buy fags.
People respond to different approaches because we are all individuals. Let him do his work. If it changes some people then what's the harm.

There is plenty of information regarding healthy lifestyles on TV, radio, magazines and internet.
99% of the population are well aware of a healthy diet and the benefits of exercise !
Whether people choose to follow this advice is a totally different matter. Many, unfortunately, don't. That's their choice. An educated decision. I am fully aware that if I choose to eat a Big Mac over an apple that I may not be making the best choice but, my decision.
I am a big believer in live and let live. Each to their own. What I find a little frustrating is when people become ill or suffer and moan that it's not their fault. Nobody told them. Just take ownership for your choices in life. I drink more than is probably healthy and enjoy the wrong type of food but my choice.
