Need advice asap

Not sure if I am posting in the correct group but I need some advice pretty quick, as I am confused about what to do!

I am on the pump as most of you know and am 32 weeks pregnant, and an hour and a half post meal...usually breakfast and or lunch but mainly lunch...I think? I am high....for example at lunch I was 9.7mmol had 40g of carbs which I know was correct, as it was literally a glass of milk (large) but not quite a pint and a small slice of bread......I had bacon as well but obviously that has no carbs! However, an hour and a half after eating I am a 15.3 but my pump still says that I had 6.6units of rapid active, so couldn't give a correction....I could always over ride that and still give a correction but I don't know what to do! At the end of the day I just want these post meal highs to go!! My diabetes consultant doesn't think my basals need to go up...which I agree with but we have changed my carb ratio at lunch time to 1u per 5g...would it be worth going down to 1u per 4g??

I am just pretty desperate as I already have mild polyhydroaminosis (think that is how it is spelt) and my baby's abdomen is already slightly on the larger side...I just don't want it to get any worse!

I have emailed my diabetes consultant but she is in Gambia and has already said that she has restricted wifi coverage.

Can anyone help please??

How far before you ate did you do the insulin? Also, did you correct for the 9.7 in your pre-meal dose?

20 minutes, I have been advised to do it 15-30 minutes before...but I was in a cafe so I didn't quite know when the food was going to come up! And yes...pump calculates that for me!

I know everybody is different. But on the rare occaision I drink milk (mainly treat a hypo.) it does have a habit of coming back to "bite me in the ****" hours later... A bit like pizza in some respects.?
 
At this stage of pregnancy you may need to look at upping your insulin:carb ratio, and it may change pretty fast. My dsn said she had known ladies to have a 1:1 ratio +5 units in the latter stages of pregnancy because of hormones!

Milk never effects me...that badly...unless it did.....sigh I don't know!

Yeah I have already changed it once, so maybe change it again???? I have just tested again and I have only come down to a 14.4, pump calculated 2.8u as a correction with 3.2 still active, so I have given that! I am tempted to put a tbr on but then don't want to hypo....hmphhh xx

I can apreciate I'm not a pumper. But is all the pumpy bits & bobs in working order..? No shifted cannula or blocked infusion thingy??

Na that's all fine, I've checked that! As I thought that was the case too..guess it could be stress aswell but honestly I don't know, I am just fed up!!
 
Change your site? Mine becomes 'waterlogged' if I use over 25-30 units in a day so will only last 2 days instead of 3 or 4. Check for air bubbles in your cartridge.
Do you have a pen? I'd correct using that but only small increments as you are not sure of your iob.
And God love you- keep testing and have the hypo stuff ready. Do you have a DSN you can contact.

I changed it this morning, and am out now...I'll use my pen and test again in about an hour or so! Might just put a tbr of an extra 20% to see if that does the trick! x

Yes I am.. Based in an unseasonably hot Loutro ATM... What with the heat. (Insulin sensitivity.) & the resterauntie guessy da carb & arival time on a da table..? I'm hitting the right balance of ordering low carb protein, (plenty of veriaty.) then picking up any "slack" with a few meagre Gs of potato or "side bread"..? Doing OK So far.... :cool:

.... But then, I'm not "with child". ;):D Congratulations by the way!

@ohitsnicola Obviously I can't advise you about your doses, but when I had highs like that in late pregnancy I corrected them (and tested lots after).

Your ratio may need upping for that meal, but don't make any decisions from one result. I used to wait till I had 3 highs after a meal to change the ratio (but I still corrected in the meantime) If you know a particular meal is giving you highs, you can have the same meal the next day and see if the same happens.

For 'dodgy' meals where I thought I might go high, I tested at 1 and 1/2 hours so I could catch the high early.

After about 30 weeks or so (don't remember exactly) my insulin needs increased a lot eg my evening meal ratio went from 1 to 10g pre-pregnancy to 1 to 4g.

So do correct if you're confident doing so. Also, you may find that certain carbs are more difficult to eat in the last few weeks eg I found I could no longer have juice at breakfast. It may be your big glass of milk was the issue.
 
I've just seen your comment about a TBR. I used a correction bolus not a TBR. That will work faster and better in my experience.
 
It is frustrating when you want to give a correction but the pump won't let you? Did you override this on the latter stages of pregnancy @azure? Just wondering?

@Sarahkylie88 No, my pump is set up for me to decide what I do. I don't use Insulin On Board or any other things like that. If I want to do a correction, I just bolus the amount I want :)

Edited to add that obviously I know what I'm doing and have a lot of pump experience - I don't just randomly do things : D
