Hi, first post. I am a type 2 diabetic - oral medication isnt working and have just been put on NovoRapid insulin. I have been told that I should take 1 unit for every 10 grams of carbs, which I am limiting as best I can.

First week and Novorapid is having absolutely no effect on my BG at all - I have vaired the dose upto 10 units before a small zero carb meal ( under supervision of a nurse) and the NovoRapid does even register.

any advice?
 
That suggests that you may have significant insulin resistance. Can I enquire of your BMI?
 
5'10 80Kg - BMI =25 roughly - 32in waist

Exercise regularly 5 times a week
 
How does a doctor decide on whether you need a basal, bolus or both......as a type 2?

as a type 1, I would always want to think about the basal need before I thought about changing the bolus insulin to carb ratio, which for you at the moment is 1:10.....

if the basal need has been considered and is being catered for then an adjustment of the insulin carb ratio is needed...........so you would need more insulin per 10g of carbs, or less carbs for every unit, however you want to put it...........

on DAFNE the next progression would be to take 1.5 units for every 10g, test, then evaluate........

As a question, when were you diagnosed t2? Have you had the GAD antibody test done? You sound as though you may be T1.5 rather than T2.
 
2003 - diet controlled, then metformin only, followed by metformin & glicliazide, recently been unable to control BG levels which prompted visit to doctor and progression to insulin.
 
Hi. As others have said I suspect you may be a T1.5 (LADA) as your weight is and presumably has been low. You don't mention Basal insulin in your post? Can you confirm that you also take a Basal insulin. It's possible that this needs adjustment. It's very unusual for Novorapid not to work and 1 unit to 10 gms is the normal start point for a normal BMI. I assume you are injecting into the stomach which is the fastest action injection area. What length needle are you using; I would expect 4mm with your slim size? A change of rapid insulin type might be needed as a few people react badly to some insulins
 
Novorapidboi makes a good point, you may need to be on basal (background) insulin also to reduce your bg levels, do ask about it when you see your Dr.
