Hi All,

I am after some advice, about 12 months ago i totally burned out throwing all my meds etc in the bin ( wrong to say the least) i was under a diabetic physcologist for help for a while also.
I was diagnosed in the middle of lockdown with DKA so the T1 diagnoses was a shock to say the least! Since then i have struggled with weight gain, i am under a dietician and have recently started to watch what im eating etc.
Im on Novorapid for during the day im carb counting also an have it pretty good (mostly green area now) for my night time im on Tresiba at 40 units,.

Now this is where things dont seem to work out my Tresiba seems to have stopped working im going to bed at around 9 but getting up and im high at 16-17-18 some mornings.

Im also suffering from lumping every jab i do lumps which doesn't help as it also blocks the insulin ive done what was said alternate more etc they also bleed well sometimes and bruise.

I dont want to meltdown again but its driving me mad have any of you had this?

Is this the novorapid or the tresiba as well?

Honestly, I think you have a case for contacting your clinic urgently and asking for help.

Have you tried modifying your doses at all?

AS a T1 who is carrying some extra weight I've found watching my carbs pretty helpful for (slow) weight loss. That plus limited quantities of alcohol and cheese.... (Got down to 70kg from 80kg over 2 years, but I prioritise diabetic control over weight loss)

Lots of virtual hugs, sounds like you are going through a difficult phase. Please try not to despair or give up, most things T1 can be fixed with time.

Im getting the lumping with both the physcoligist im under mentioned an i port to help with less injection sites but i would need to get funding....my diabetic nurse said she doesn't even know what it is!......

Yes i have tried modifying the dose even splitting my novo in half.

I am trying now to cut out doses of insulin to try see if it makes things easier (not eating carby foods) a diet shake for breakfast an apple at dinner and soup for tea as the weight issue is a biggy its causing other problems to!

I feel like i have been deserted by my diabetic team in some ways after a bad spell last year i think they just dont want to offer the help but that could just be me also but thats how they have made me feel.

A bit of a catch 22 for me at the moment trying to do right getting my numbers down etc then extra issues.

This could be related to what you eat for supper/in the evening and whether it matches the insulin you're taking - especially for larger meals which contain fat/oil which are slowly absorbed. I spent a long time fiddling with basal dose trying to reduce overnight highs, but in fact they were due to not dosing correctly for supper.

Do you have a libre or other CGM, can you share some data to show what the day looks like?

fwiw this isn't just for the night time, it's designed to work all day and in the absence of food should keep you stable. It's generally more important/tuned to match the night time part as you're asleep and want it to just work, whilst during the day you can make manual adjustments (more insulin, a snack).

I dont do supper i have my evening meal at around 6.30pm to which my carb counting has been pretty accurate.
I do have a libre but i haven't a clue how i would share it here my numbers in the day have been pretty good mostly green zone occaisionaly going to about 12.
i understand it isn't just for nighttime sorry i should of made it clear that it is when i do it.
 
What time does it start rising? Could you post a screen grab from your phone which shows the evening/overnight period?

My regime is so different to yours i wouldn't know where to start.

However however

If you are mostly in the green during the day you are doing fantastically well.
Dwell on that thought considering all the extra problems you are dealing with!!

I know you need to get the night times improved but you evidently understand the data thats coming your way.
