Questionnaire for a school project

Hello! I'm a high school student in South Africa. I have to do a project on diabetes, where I'm required to 'interview' at least 40 diabetics to gather enough information to present graphically. It would be a huge help if you could please fill out this questionnaire for me, or fill it on behalf of anyone else you know with diabetes! I don't need any super personal information such as your name or where you live. If you have any questions or think I can/should improve my questions, please let me know :) Thanks!

Is your project looking for anything in particular or does it have a particular purpose?

You may want to change your question about Cause. Type 1, for example, is an auto immune disease and its cause is unknown.

Hey @azure It's just sort of a general project on diabates, as in what it is, functions of the pancreas, biotechnology etc. I need to set up the information in my questionnaire in a graph and discuss my results. It's not very specific.
And do you think there's a better way to word that? Otherwise I think "auto immune disease" is alright as an answer on its own.

It's up to you, of course, but I'd either amend it to ask Auto immune or not or something, or remove it altogether.

The answers you'll get may be too unspecific. It depends on your needs.

Or maybe just add "cause" (if relevant)

Cause - Possibly due to obesity

Cause: reactive hypoglycaemia, prolactinoma, polycystic ovary syndrome, medication, obesity. Could be any, or all of them. They are all interlinked.

Female 59. White British. Type 2. Probably caused by insufficient Thyroxine causing a shortage of insulin being produced and causing my pancreas to fail. Diagnosed March 2016. A newbie! Good luck with your project, if you need any support let us know! I am an English teacher!

Cause unknown - I am not excessively over weight and followed a fairly healthy diet . Could be linked to stress / depression.

Your age when you got it: Diagnosed at 50, possibly had it for at least 2 years prior to this. Definitely 6 months.
Cause: Probably a combination of genetic disposition and weight.
