Just to clarify, your doctor/lab tests show a yearly increase of 0.2% in HbA1c?
May I ask what the last two x yearly HbA1c results were?
It is said that some people can have a low measure of IR without going on to develope Pre Diabetes or Type 2 Diabetes.
To be honest I think that you are doing everything you can to guard against insulin resistance by lowering your carb intake and excercising. There is one area that you may like to work on and that is stress/anxiety levels.
You could try reducing your carbs further perhaps?
I have felt lack of energy and had bad bowel movements on ketogenic diet. I feel better on 100g of carbs, mostly from beans, peas, carrots. I think it is worth it because I am much better on this diet than I did on ketogenic for a year. Quality of life is a high priority for me as well.
