Seems strange...

There appears to be a pattern emerging for me...

Can anyone shed any light...

Post meals bg is 5-6 generally

I eat (tonight = sandwich, salad, chicken) I then tested two hours after the meal and I am 4.2 and on the way down (shaky, sweats etc)

If I test soon I will be 4 or under...

The same appears to happen after lunch - with the low comes extreme tiredness period that lasts for hour

Can you please tell me what medication you are taking. Please include all medication including those not related to Diabetes. Either on the board or as a PM.

Yes you sound like you are now overmedicated. This can occur when your diet causes you to improve when they are expecting you to get worse.
 
I'm interested to know how the right level of medication is determined.

At the moment I am on 2 x 500 Metformin and 2 x Gliclazide per day, but the only way I can even approach the 'normal' range of blood sugar is if I eat my own weight in cucumber and dig for 3 hours between meals, and obviously I can't keep this up indefinitely.

Any normal pattern of behaviour in terms of eating/energy expenditure and my BS goes up into the teens after meals, so I'm wondering if I need more?

Talk to your healthcare team and ask if you can reduce the dose. I they say "no!, eat more carbs!" I'd just cut the Gliclazide down anyway and keep checkng for safety. But that's me and I'll take the consequences of what I do. In any case, Gliclazide tends to make you gain weight or at least make it hard to lose.
 
Hmm.. how does that work? I'm not sure what effect that would have? My doctor told me the glic kick starts the pancreas to make more insulin, and the met makes the cells more receptive to the insulin, so presumably the two working together helps the body to make better use of the energy it's getting?

If I cut down on the glic, would I have to cut down on carbs? I tried that last week and was practically suicidal, so that's not a solution for me. I'm not eating much carb even now, but can't cut down more without either starving or throwing myself off a cliff. Neither of which appeals.

I have lost a bit of weight even so, but it has been hard work.

that is what glic does. It's not regulated by the amount of glucose in your blood though. It just keeps kicking until it's worn out. If you want something that is self limiting, ask the doc about Starlix. that is better. It's theoretically possible for glic to thrash the pancreas to death and leave you with insulin as the only option.
I got off glic totally, by low carbing. If you don't take in the glucose(carb), your system doesn't have to deal with so much. In any case, if you cut your carbs a bit and monitor, you should be able to reduce the glic. Ask the doc. You will need strips.
 
I have got the strips and am testing, that's how I know what my sugars are.

I have cut out loads of carbs, but still struggling to keep within range and eat enough to stay alive. :( I can't eat much at mealtimes, so get hungry in between, and snack a bit which means that come mealtimes I'm still too high to be able to eat very much, it's a vicious circle.

Would I perhaps benefit from more metformin? What are the criteria for putting it up?

When it isn't working?

Many people need to go up to 1500 - 2500mg for it to take full effect, you are usually started slow and walked up to reduce brown trouser incidents. Take your numbers back to your doctor and discuss.
