Set change this morning now BG is high!

Hi

I did a set change this morning (only the second time I've done it unsupervised) and all appeared to go well, no problems. I had breakfast (took bolus) and tested an hour later and I was 11.

I'm now out for the remainder of the day. I sat down to my lunch, tested by B.G and I was 14.4! This is the highest I've been since going on the pump, and all previous lunch time readings have been pretty steady from my waking readings.

So, I corrected using my flexpen but bolused using my pump (not sure if that was a good idea or not) and I had my lunch. I'm hoping it's a blip but now I'm concerned I have a set failure of some sort. I do have a spare set with me. Any advice on the best course of action?

Did you remember to prime the set?

Does the set feel comfy?

Is it the insight you are on? If so I found that the tube fill and the cannula settings they say to fill with aren't enough...
Apologies if you aren't in Insight, I lose track on which pumps people have nowadays!!
 
To be honest though if my sets are to blame then I would be in 20's within 2 hours....if I had ate as well then I would be nigh on comatosed in 30's by 2 hours....
 
I'm on the medtronic mini med.

I primed it and insulin came through. The set felt a bit sore going on (but seems normal for me) but after a couple of minutes was fine.
 
You can only monitor now...personally if I go to 14 which is very rare I would change set again
At my earliest time possible.
 
If you press against the set can you feel any discomfort now?

Ie my sets go in my bum so I am asare at night I will have them pressing against the mattress. If they are sore when I do that then insulin isn't going to go in properly. So I always just press against my sets when putting them in, if i can feel them at all, then I quickly take them out and change the place.

Oh dear

Its difficult to know whether the high bg is due to set not inserted correctly or whether the basal needs adjustingasvthis does happen. Obviously in panic you delivered the bolus on the pump but also used yr pen as well what you have to do now is bg test every hr to make sure you dont go low and eat some glucose stuff or jellybabies etc to do a quick bg rise. Only eat some carb if bg is on 6mmol and going down but even then you may need to eat something sugsry.
 
No discomfort when I press on it. Can't feel a thing.

I know iHs, it's an awkward one. My BG on waking was 5.7, so nice and normal.

I'll make sure I monitor hourly. If in this situation again, what should I have done with regards to the bolus/correction?

On a really high bg....I think its best to use pen than chance being high for many hrs waiting for a correction from the pump to work which might take ages. Next time if megga high, use pen, avoid keytones and once bg has lowered after 3hrs, go back to pump again and then you'll be able to see if the problem was due to set or whether the basal wasnt correct.
 
I can remember reading about problems with quicksets years ago. What might be happening is the circular adhesive of the set is transferring some of the stickiness on the blue inserter as the set is fired into the skin. What some did, was wipe the inside of the inserter every week with an alcohol swab to get rid of any zdhesive residue. The other thing is to make sure the set is in the inserter ok before firing.
 
It's been a 1hr 15mins and I'm test again and I'm 17.2

Bearing in mind I had my lunch which was was 45g carb and I bolused 4.5u with the pump and 2u correction with pen.
 
If I was you I'd put my pumo on a temporary basal of 110% - 120% to see if that helps. If that brings it down it may just be a time when your basal rates need adjusting although coincidental that you did a set change this morning. Some insulin must be getting in to your system though as you'd be higher I think by now. All the best
 
Thank you everyone for your help.

I got through to my nurse who told me to just change the set, and wouldn't you know, the cannula was bent double! So, it was a set fail after all.

My BG went to 19.1 and ketones fairly high. I've been chugging water and they have started to come down and given myself an extra correction dose with flexpen as I delivered lunch bolus via pump.

My BG before lunch was 14.1 and right away I figured something was wrong. All a learning curve, I guess. I have cancelled my evening out as I don't feel very secure being out with BG/ketone monitoring, etc.

Only my third set change and already a fail. Are they quite common?
 
Glad you've got to the bottom of it - the pump is a steep learning curve :)
 
Its a definate learning curve alright but now you know lol. If a set is up the creek then although the pump wont always throw the occlusion warning,the pump will only deliver very tiny amounts and bg doesnt really come down unless a pen is used or a highish TBR but usually if a pen is used,more of a correction is needed than what the pump may recommend. Bg testing reveals whats wrong. Glad things are ok now

Down to 11.1 now, so feeling better.

All the advice is really invaluable as a new pumper. Plus it's one way to get through all your test trips, lol.
 
If you can remember thst is, try to insert a set just before eating and do usual bolus. If bg goes to 12 to 15mmol 2hrs later, then its either carb count not correct, bolus not rught rstio or basal needing sdjusting. If very very high correction with pump might need increasing for good measure but if that doesnt work, then for sure its insulin pen and increased correction and replace set when possible.
