Type 1 Spike in a morning?

Hi,

I wondered if anyone could offer some advice/guidance.
I have been T1D for nearly 10 years (diagnosed at 14 y/o).

I am currently on novorapid for meal times (ratio of 1-10) and glargine on an evening (30 units). Recently, my blood sugars have been dropping in the night, this has been rectified and my blood sugars will sit around 5-6mmol throughout the rest of the night, but then spiking first thing in a morning (up to 16 mmol). I have tried dropping my glargine to 28 units but still seem to be having the same issue (hypo in the night, spiking in a morning). I will correct my high bloods in a morning by giving myself a large correction dose (sometimes up to 5 units, but my body seems very resistant in a morning).

I have trialed giving myself my glargine on a morning to prevent the nighttime hypo and morning spike - however, I am still having the same issue.

I have now tried giving myself half dose on a night and half in a morning (15 units am and pm). Last night my bloods sat at 5.2mml throughout the night. I woke up this morning having a hypo (3.8mmol). I ate 1 chocolate digestive (10.1g of carbohydrates) to bring my bloods up slightly. After 20 minutes, my bloods were reading at 16.6mmol??

Could anyone please advise how I can manage my morning spike as I’m at a loss of what to do??

Hi @abbylgerrard and I feel your pain.
The dawn phenomena is tricky.
I split my lantus and reduce the dose till I no longer have night time hypos, but I do tend to have a dramatic spike in the morning, countered by injecting extra bolus. It works for me, sort of, but I do tend to spike in the morning.

Would you consider going on a pump? My team are finally putting me on one (combination of hypos and dawn phenomena) ?

It might also be worth asking your team if a different basal would help?

Were those readings from a cgm or a glucometer? If the 3.8 was inaccurate then that could explain the large increase? Having said that, if you had hypo symptoms then the 3.8 was probably accurate, though the 16 might not be....
 
Helloo. Thank you for your response! Im trying to trial playing around with my glargine, but unsure how to prevent the spikes in the morning? I give myself corrections but it’s like my body doesn’t take to it in a morning. I’ve never been a fan of the idea of a pump ☹️ thank you for your response!

Many people find their ratio is very different depending on time of day, and so are the corrections needed.
I need a lot more insulin in the morning than in the evening, be it for food or for corrections, it may be worth looking if you have something like that going on as well.

I also tend to spike after waking up, even if I'm stable throughout the night.
Because this is a quick spike, caused by glucose dumped from my liver, I use quick acting insulin for it, not long acting.

I’m the same! I spike upon waking, even after staying consistent throughout the night. I give myself a correction but I don’t always feel my body takes to it?? I’m not sure what I can do to prevent this? Thank you for your response!
 
Do you get this spike consistently every day?
Do you correct after your BG has risen or before, like you would with a meal?

Yeah every morning. I go to sleep with my bloods around 6mmol then it spikes to 16mmol without fail. I tend to do it once it’s risen as it quite literally spikes as soon as I wake up

How long after waking up do you inject for it?
You say you spike as soon as you wake up, so say you're at 6 still right upon waking, how long does it take to rise to 16?

That's exactly what happened to me if you can't fix the hypo by lowering the glargine dose I think u may have the same problem I got 3 months ago I got hypo even if I injected as little as 6 ui glargine tried everything lowering the dose split the dose take it at night morning or evening( my normal dose was 25 ) I tried every possible solution so I've come to the conclusion that the glargine itself was the problem told my doctor the glargine was the problem and I want to switch to tresiba he told me that impossible glargine will not do that and refused to switch me so I just went to the pharmacy bought it inject it and everything worked perfectly no more hypos
