I'm going to be switching from lantus to levemir after a meeting with the DSN for my KAT1E course (kent's version of DAFNE). Currently I am taking 10u of lantus at 9.30pm, the DSN recommended taking 9u levemir first and see how it goes before splitting it. What are your experiences of this? How do I go about splitting it and when should I make this decision? Thanks in advance!

with a switch of long acting insulin best bet is to follow your nurse advice and test results over a few days before making any changes.

also some basal testing to see what you are doing overnight to try and split the dose to meet your needs.

When I briefly changed from lantus to levemir several years back my DSN told me to sart on the same dose too, however she did warn me that people who make the change usually find they need a third more levemir than they would when using lantus.

I didn't fair well on levemir, ended up doubling the dose to what I was taking on lantus and still couldn't get levels under control, this despite trying split-dosing working on various splits.

The reason my DSN suggested the change is because my hormones play havoc on my readings and so with levemir I can adjust daily, she also said I could have a half unit pen which would mean I could make smaller adjustments. I am okay on lantus, it does sting a bit but overall I'm okay. I just can't adjust it quick enough to fit my hormonal changes!

Hi. Interesting your comments about Lantus versus Levemir. I'm surprised you can't tweak the Lantus as easily as the Levemir? I'm on Levemir and find it doesn't sting at all. It doesn't last for 24 hours which is why many split the dose. I don't split and just tweak the Bolus ratio as needed. It's easier to tweak the Levemir if it isn't split.

My nurse said lantus takes 3 days to adjust to any changes which isn't quick enough! How long does levemir take? My nurse told me it's the same day so a lot more flexible.

@asyarik, Levemir is out of your system within 24 hours at normal levels. That's a blessing and a curse of you're used to Lantus.

I find Levemir to be brilliant because of that ability.
