Switching from Toujeo to Tresiba

After bad hypo last week on Toujeo I am switching to Tresiba my DSN advises to use same dosage and take at night. My one concern is that Toujeo is meant to be 24 hour lasting. Tresiba information states lasting 40 hours.

I have doubled checked that I should stopsToujeo and replace with nightly dose of 22 Tresiba I am so worried that if it has a lasting effect of 40 hours I will over dose or insulin stack. I'm getting a glucagon pen for back up but feel very scared.

Any advice appreciated

Hi there , i converted from Levimir and had the same concern. My DS reasurred me that the levimir would be moving out before the treseba moved in properly. It does take ( i found ) 3 days or more for the dose to reach its potential. Keep checking those bloods id be more worried about elevation . But im not a chemist only a mere pup t1 10 yrs only! Some experienced folk out there will put you on the right track. Test test test youll be fine i found the transition easy then the fun of getting the basal right began ...

Thank you so much for your reassurance I am very worried so trying to put measures in place in case of hypos. Have requested glucagon pen plus will have plenty of sugar to hand in case I nose dive like last week. Fingers crossed.

How are you getting on with Treseba?
 
If you google pictures of conversion of Lantus to Toujeo, and Lantus to Tresiba, the conversion dose of Toujeo is 80% of the Tresiba dose. So that may need to be taken into account as you stabilise further.

Very well thankyou infact im handling this monster very well compared to 12 months ago! I run on a steady 6-8 , eat under 10 gms carb through the day then a nice meal at 6 ish with a small bolus of 3 or 4 so not bad night is a prob i need less insulin at night so i do tend to be to low for my liking 5..at 4 am but i just swig some milk and take a bite of sandwhich ( both prepared earlier ) from bedside and back to sleep. I dont want to change insulin now. I do find that any change of basal takes 5 days to see a differance... honestly it really is that long. So if you excercise
I guess this wont suit some people . Im pretty sedentary now im retired good luck youll be fine relax and test !
 
Hi @Tracie1212 When are you due to switch over ?

I can appreciate your anxiety about the switch, the profile of toujeo is slightly longer than 24 hours, it can take up to 30-36 hours to deplete from your system. As @kitedoc has suggested starting on a lower dose would be wise, your DSN would typically start you on a lower dose of tresiba for this reason and you would then gradually increase your tresiba dose over a few weeks until you're really settled in as tresiba lasts approx 36 hours then it's advisable to make any dose increases every 3-4 days to see the adjustment settle in.

Call up your DSN again and run through the changeover with them first to confirm this if you haven't already switched.
 
I have spoken to two DSNs who both tell me to use like for like doses. When I ask further about insulin I am taking they say that Toujeo is a new insulin and don't know much about!!! . Not much advice with Tresiba I did ask quite a few questions but got similar reply to Toujeo handed a leaflet and advised to stay in touch with DSNs which doesn't instill a great deal of confidence. My severe hypo was at night and they work days. I've not started yet as want a glucagon pen in the house first my request is in to doctor for.
