Hi All,

Here is my story:

I am 35 years old, athletic body type, South Asian living in Europe for last 14 years. Being surrounding by families of T2 diabetic (Mom, Dad, Aunts, Uncle, Grandfather) I am ware of diabetes and its complication. So I kept myself busy at gym and sports, from 16 till 34 years. Last year i was not regular at gym and running, still did some exercise. And also I did yearly blood checkup for the past 10 years and all seemed fine until this Jan 2018. My results came HbA1C - 8.3 mmol/L and 2 weeks after my clinic wants to double check and it was 9.8 mmol/L. I have lost 4 kgs between Jan - March. My fasted random BS - 12 mmol/L and post meal was 18 mmol/L. I was asked to go to hospital and there they tested my pressure, blood and urine. Already by this time I was eating low carb and balanced diet and exercise. Hospital results showed normal pressure readings, random glucose - 16 mmol/L and ketone 2+ in urine. Doctor explained ketone are due to my low carb diet and I should worry about it. I was asked to give fasted blood to test C-peptide, GAD65 and C-reactive protein and I am put on metformin 500mg in breakfast and dinner after 2 weeks increase in dose 500mg*2 during both meals.

I responded very well my average random BS dropped to 7.5 mmol/L for 3 week period and Nurse mentioned that rest of the results have come and it doesn’t look good. my readings for C-peptide (494 pmol/l) , GAD65 (500 above) and C-reactive protein (0.7) and this means I am LADA :( and this is devastating for me. I will meet the doctor on Tuesday.

My question:

For the last 3 days my random BS after 9 pm is shooting over 10 mmol/L for both fasted and post meal (14 mmol/L). I still have same dose of Metformin and also today after 45 mins of workout and running and after 30 mins of resting my random BS is 10 mmol/L it was usually around 4.7 mmol/L over the last 3 weeks. I see a pattern over last 3 days in the increased value in the eveing and night. I am worried and curious to know if anyone in the forum have come across this? Also I feel some sort of burning sensation in my tummy and upper abdomen, I have been reading too much in the web and i fear that my Beta cells in Pancreas are burning out as I am LADA. Is this symptom common to LADA ? or is it bcos of Metformin? or its just my imagination ?

And also all my family is T2 and is it possible that I could be T1.5? My family doctor said its possible and family couldnt believe it. Both my Uncle and Grandfather were diagnosed as T2 at 33 years old and is it possible I m T1.5 at 35.

I have been reading this forum for a month and I am a newbie here. Thanks for reading. Your comments are greatly appreciated.

Hi and welcome. First as the indication is that you are LADA you will need to go onto insulin at some future point when you can't control your blood sugar. You will currently be in the so-called 'honeymoon' period. Your beta cells will be fading and I'm afraid that's the luck of the draw. Many LADAs like me have no family history of diabetes. Be aware that metformin never has much effect on blood sugar. It helps just a little bit but GPs/DNs tend to rely on it too much to 'manage' diabetes. I still take it as it helps just a bit and has other protective properties. It is a very safe drug. Your doctor was only partly correct ref low-carbing and ketones. Extreme low-carbing can result in ketones (Ketosis) which are not harmful as long as your blood sugar is near normal. If you have ketones AND high blood sugar then you can have Ketoacidosis which is very dangerous and can be fatal. The latter happens with in-diagnosed T1s not on insulin. Many athletes run in Ketosis as part of fat-burning routine.

Hi Daibell,
Thanks for your reply. I am not very active in the forum.
My HbA1C is 6.5% in September and my endo doesn't want to put me on insulin straight away. Lately I have been struggling with high fasting morning BS 10 mmol/L. Hence after discussion with endo and nurse they put on Tresiba 2 weeks back. I started with 6 units and now on 8 units my morning fasting BS dropped to 6-7 mmol/L. Now I have no doubt I am LADA and my body need insulin, happy to be on insulin and also take metformin. Lately i read good things about metformin reducing cardiovascular risks.

I lost 14 kilos over the last 6-8 months. Can I gain my weight back in ketogenic diet?
