More from me today sorry! More questions keep cropping up as I get used to the pump!

I wanted to test the extended bolus function on the pump today, so we treated ourselves to pizza for dinner.

I’m on a high ratio of insulin due to insulin resistance. My meal insulin plus a correction exceeded the max bolus of 25 units. I worked through the various options, expecting the pump to allow me to override the max bolus.

The pump didn’t allow for this, so I’m stuck with a bolus of 25u which is a bit short. Trying to add a top up is confusing as I can’t see the original bolus calculation now that the other bolus has been delivered!!

I’ll call air liquide to ask in the morning but somehow i feel like I’m being told that there’s a limit to how Carby my meals can be now.

II would use an insulin pen to add to the bolus. Today I did just that. I was out at lunch-time; was trying to give myself a bolus of 2.88 units but the pump only gave 0.66 and I got a malfunction message. I made the bolus up with a pen. Most of the time my Tandem pump work well but just occasionally I get a blockage warning.
It will be interesting to learn what advice you get from Air Liquide.

don't know your pump but on my pump you can adjust maximum bolus in the settings --- have a chat with your nurse

Yes you would need to change this in your pump settings under personal profile, I can change mine which is currently set at 10 units and was set up with my tandem rep.

I’m on the t:slim x2 and have already increased my max bolus to 25u (currently their upper limit). I’ll try to find out if there’s a way to override this or if you have to just figure out the correction manually.

Ok so if that's the max bolus limit, then why don't you do 2 boluses, one for your 25 and the other for what ever is left ?

@Juicyj that may be the solution! I find it all a bit confusing- by the time you get through all of the warning messages about max bolus you’re about 5steps away from seeing what the bolus was supposed to be!

Then you need to work out the value of carbs to the amount of insulin that’s missing. Also at that level of bolus, I’ll be running an extended bolus but you can’t run 2 at a time!

I think the next time, I might do a 40:60 extended bolus and try to add the extra on as an additional bolus straightaway so it sort of acts as a 50:50?

My brain is imploding at the thought of it all lol! Maths is not my forte! Maybe this is something that was just easier on pens?

Answer from Tandem is really confusing me.

They are telling me that I should receive a prompt to deliver any remaining insulin once the bolus has been delivered. I didn’t see one the last time I tried to give a big bolus.

I would never use more than 25units without using the extended bolus feature. Tandem can’t tell me if using the extended bolus feature means that I won’t get the prompt.

I rarely need to use this much insulin for a meal - mainly things like pizza/eating out.

I’m really disappointed in this feature of the pump as I thought the pump would help me to manage these types of foods better.

I have a really high level of insulin resistance and I have no idea why. My clinic constantly tells me how surprising my insulin resistance is for my build/diet.
