What a shocker ,

Yesterday I was with Bro and family, we do this each year; a great family day with Christmas for the grandchildren. I spiked at 13.7 two hours after dinner and at bed time was 17.3. BGs this level are not unusual for me a LADA diagnosed in Oct 2013.
Today I have felt unwell, very tired, put that down to yesterday with the youngsters who include me I all they do - and I love that.
Unusual, insatiable thirst and off my food forcing enough down to "manage" my Humalog mix 25 at breakfast and meal this evening. My BG just prior to eating this evening displayed HI on my meter with keytones flashing. I waited 10 minutes and retested, it was down to 25.4 and the keytone test was neg. I have sent an email to my DN.
My last A1c in Oct13 was 71, BGs since never lower than 8 even after fasting and highest most days is between 15-19. I have autoimmune problems with many of the associated autoimmune problems.
Is there anything else I can do this evening or need to do this evening ?
I keep hearing that LADA is,difficult to manage and I must agree.

Thing is that lada is progressive,.eventually leading to full type 1, sort of prolonged "honeymoon period". Maybe it has progressed enough, and ned to adapt meds to suit. With having those levels regularly maybe discuss with dsn a basal/bolus regime which is more adjustable to your current needs.

Many thanks Brett. My Humalog Mix 25 has been increased several time a since I commenced in Oct13. I'll await her advice and take, it from there.
I reached this current stage in 5 months having had great A1c results for many years and well controlled day to day. My autoimmune system is battering me At the Moment. My thyroid is low output and coeliac disease is also possible. These will be addressed once the diabetes is more stable. I again, many thanks much appreciated.

Alan..... May I ask why you " forced enough food down" to manage your humalog mix 25? When my BG is high and I don't wasn't to eat....I don't. IMO its my body telling me it doesn't need food. I take my humalog25 mix as usual, drink lots of water, don't bother about food and allow the insulin to deal with the excess glucose. For me, it works.
 
Only suggesting that cause op is lada and taking a mix of long and short acting not eating could result in a sudden drop, even if high causing a hypo and has probably been advised by med team to always eat when taking. As there is no resistance to the insulin as well as an erratic insulin producing pancreas its a total nightmare. However, someone with a bit of resistance could probably go longer without food after taking. if he was on long/short acting separate more adjustable to then miss meal without taking any qa insulin.

Yes, I understand. However if someone is experiencing such high BGs is it not better to allow insulin to work alone and carefully monitor BGs for a sudden hypo? Personally I'd prefer to take the chance, but it is just what I'd do.... I'm not recommending it for anyone else.
 
Hi Alan!

I really sympathise. I am LADA and have been for 4 years. You hear all the time that a little treat won't hurt you - unfortunately with LADA it probably will as you have demonstrated.

I agree with Brett that you would probably be better on basal/bolus so you can adjust the amount of rapid acting independent of the basal dose.

In general, your numbers are too high and your diabetes team needs to take this seriously - you are in danger of complications. I would say cut the carbs from your diet to control LADA but you can't do that while you are on a mixed insulin or you will hypo.

LADA is difficult to manage, but it is possible. My last HbA1c was 5.4% on a basal/bolus insulin regime with a low-carb diet.

Remember, you are not alone in this. Most LADA's struggle for good control - even with my tight control I have frequent spikes into double figures. Three of us well-controlled LADA's did an experiment on another forum to test our control over a month, publishing and graphing our results. I wish I was allowed to point you to that because it would show you how we manage and the difficulties we all have.

Anyway, please try to be sensible with the food for the rest of the holiday period and see your medical team as soon as you can. Drink plenty of water when your BG is high and when they are lower, try to increase your exercise - maybe a walk or something.

If you have high BG, ketones and feel ill, go to A&E immediately.

Yes, I would ask for a Basal/Bolus regime which my DN has put me on. She started me with just the Basal but added the Bolus after a few weeks when I was spiking at meals. The only advantage of an insulin mix is you only have the one insulin and pen and fewer injections but the total control that Basal/Bolus offers I feel is worth the small extra hassle.
 
Many thanks indeed to all - I'm just such a beginner at this LADA having been diagnosed incorrectly as T2 in 2007 and according to GP was probably diabetic for 30 years prior to that being masked by other health matters and drug therapy. No matter, I am where I am and doing my best to get to grips.
My diet has changed beyond recognition, thank goodness and am working on that. Strange to me, that some foods will spike me one day and not another but that may be due to the fact that I am making some little insulin myself on some days. I keep an XL of all food eaten along with Carbs, Cal, Protein, fat, fibre and sat fat and have sent the latest XL to my DN today. the XL also averages over 7,14,21 and 28 days. I had email from DN this morning and she is not alarmed but will make judgement, amend insulin etc when she sees my charts. My food intake is 50% of what it was end Oct 13 but I have yet to lose any weight. May have complications with this due to other health problems some of which are autoimmune. I'm going to try and get my drug regime loaded into my profile today - interesting reading that. Just hope the server doesn't run out of space.
