When a person is titrating the basal insulin upwards.
Is it the first fasting measurement you're increasing the insulin for? Or are there others? Other fasting numbers at other times or both?
My health care team hasn't time for my questions.
For example, are we only looking to get to 4 to 7 fasting as our first measurement when we get up? So thereby increasing basal until we reach that?

Hi @cdpm there are unfortunately not that many regular posters who are T2 on insulin, and the T1s like me are possibly a bit nervous about giving you T1 advice rather than T2 advice. (Specially given that forum rules say we can't give medical advice). How many injections of basal insulin are you on a day?

Do your health team respond to emails? You could ask them for a link to their recommended way to do basal testing.

Naively I would have guessed that doing a test when you wake up would be enough, the important thing is to not have eaten for a good few hours before testing and to be consistent about when you test. (Others may disagree?)

The link below just says "before breakfast"?

Are you still using a cgm or fingerpricking?

You'll monitor your fasting blood sugar and adjust your basal insulin dose accordingly until it's within a target range. Your personal goals may vary, but the general approach involves gradually increasing the dose while monitoring for hypoglycemia.

Both. As the sensor area takes around 2 to 3 weeks to heal in between sensors.

At the time of starting 1 basal injection of Xultophy 8 months ago my morning numbers were in the low to high teens.
My instruction from the nurse was to start on ‘x’units and increase by 2 units every three days until I got my numbers down to between 6-7. She said not to focus on a single daily number that was higher/lower but to look at the average over the three days.
Once I got stabilised to between 6-7 I could then look at tightening control a little.
I got a free trial of both Libre2 and Dexcom 1 and found them to be useful in controlling my carbs.
Once I got control of my carb intake I was able to reduce my basal insulin by 6 units to maintain my control.
I hope this helps and sorry you’re not getting more support.
Does the instructions that came with your insulin not give some help and guidance?
Best wishes as you embark on this journey.

So I've decided to use the first test when I get up.
Thanks for all the replies.
