Which new pump as a 4 year replacement

Hi My pump is due for renewal.

Currently use the medtronic 640g. I also use Libre Lifestyle 2 funded by NHS.

Options offered are medtronic again.

Medtrume - but looks too big and cumbersome. I might look at the Nano when its out in August but fear the insulin wont last more than 2 days max. Not keen on these big attachment pumps mounted on the body.

Ypsomed pump

T-Slim with Dexcom CGM (would require change from Libre2) but looks a great pump. However the sets look like they are medtronic sets?
UPDATE - NHS won't fund Dexcom at the moment and I cannot afford £160 per month. So unfortunately this won't give me anything more than I have currently.

Are there any half decent reviews on these/comparison pages? Think I might sit on this to see if Dexcom becomes available anytime soon.

I am not sure why you think it is big - it is smaller than Medtronic and T-Slim and the only patch pump you list.
Medtrum have a new Nano pump due out in the summer. It is about 45% smaller than their current pump in a more rounded/ergonomic shape. I have seen photos but not held one yet.
Anyone on the current pump will be automatically upgraded to the Nano.
 
I knock off my libre sensors too easily let alone a pump!!

But you don't put a pump on your arm if you think you are likely to knock it off.
Unlike the LIbre, patch pumps are allowed to go in other places. I put mine on my abdomen and love having a smaller pump without tubes.

I have the tandem it’s a nice pump.. it can be used without the Dexcom.. but should the nhs change their mind.. or you can offered the odd sensor the you’d have the benefit of being able to use it


I’ve not had any other pump so can’t comment on those

Don’t hold your breath for the nhs sullying the Dexcom though
 
I was also using the 640g and am now using Tandem Slim with Control IQ which is linked to the G6, it has been an eye opener using this pump, simply because it does my basal adjustments automatically, and since using this pump my levels during the night have improved massively, so I stay in range during sleep now, no hypos, good quality sleep impacts on mood and daily glucose control, so of great benefit. Only downside is that I like running and I find even in exercise mode which I set an hour before heading out, that I can often go hypo so am having to eat carbs before evening runs, minor issue but I run with others so hate slowing others down if going low.

I am also experimenting using sleep mode 24 hours a day which keeps a tighter range and this has again improved my daily time in range by 10-15%.

In your shoes i'd take the tandem slim and if you can self fund then get the Dexcom G6 at £159 a month, for me it's been the best switch in all my time as a t1 which is about 9 years now.
 

thanks I think this is where my head is at. I've even asked whether NHS would partially fund or give me one box every 2 months...I will also be speaking to my eye consultant to see if he'd support an application for funding. Crazy if not as they fund Libre2!
 
How long does it take to recharge battery? Looks like you have to connect it to a charger?! Does this mean you have to sit plugged into a charger every day or every few days?! Is there an emergency replacement AA battery that can be used if you run out?

The pump gives you an alarm when it hits 20% and you can either take it off whilst in the shower or bath (not waterproof unlike the 640g) and charge it then as it takes about 30 mins to fully charge or sit there whilst on the computer and plug it in then whilst still connected, however the alarm will wake you if you're asleep so it's best to keep an eye on the battery. It was a minor inconvenience compared to the 640g but it's quickly over ruled as a negative based on the positives of using Control IQ.
 
Sorry forgot to mention it needs charging about twice a week, and thanks for reminding me, my pump was down to 20% !
 

Speak to your DSN about it, I doubt they would partially fund it - it's all or nothing, your relationship with your team is vital for getting support, I have a brilliant DSN but have built this over time through regular contact and trust, I wouldn't have the tools I have today if it hadn't been for this.

Can you self fund the Dexcom G6 ?
 

I think the good relationship goes a long way

They wouldn’t part fund or do an every 2 month too complicated for them to manage

The libre 2 costs the nhs a lot less than the Dexcom which is why they provide the libre

Again when G7 comes this may change

Get the letter of support etc.. and ask your dsn how you can appeal the decision. I think it’s the CGC? You can appeal with. Supply the support they have the power to overrule the decision. But it may still be turned down

I charge mine daily whilst eating breakfast, it takes maybe 10 mins at most. If you drive to work you can charge in car too.
 

I would second this as I'm in the same boat as Juicyj. I self funded for around 10 yrs and now fully supported.

how did you get support? I've looked at the current guidelines and it only says about loss of hypo warnings, pregnancy and kids.
I've got issues with eyes so I'm hoping my eye specialist will help.

It was because I’d self funded that my team was prepared to take on my Dexcom prescription with the Tandem Slim as they could see I was keen to invest in my own health and put the effort in to make the pump work, I wouldn’t of got this otherwise.
 

Very similar to @Juicyj, luckily I live in a county where the CCG will offer some support for Dexcom where my consultant and hospital are based in a next door county that does not support it. The team pushed really hard for me to get funding and its my understanding that they were initially turned down a few times before the request was accepted.

It was driven by a change in financial circumstances after self funding for about 10 yrs (together with a move to t:slim) which made it much more difficult for me, then together with the benefits it provided weighed against the impact of loosing and reverting back the 10 yrs of improvement. It was quite hard work for the team so as mentioned before they will need to put in a lot of effort and may not alway be successful sadly.
 
Out of interest do you have nay top tips for tslim plus dexcom? I have managed to get both funded by my CCG and am looking forward to getting going mid July having heard good things from an acquaintance.
