Hi everyone
Diagnosed type two 5 years ago. Prescribed 2 x metformin per day. 500mg each.
BMI at diagnosis was 24. All other stats like blood pressure, cholesterol etc. were fine.
With low carbing the metformin was cut to 1 per day.
C1 was always 7 or below.
About a year ago I tried the Newcastle diet even though I couldn’t really afford to lose weight. Thought it may reverse symptoms.
It did for a while and DNurse advised me I no longer needed any metformin.
Roughly three months ago I noticed a rise in blood sugars so started on 1 metformin again. Saw nurse who agreed. Sugars continued to rise further so I went onto two per day. I then went onto three and then four; the maximum dose of 2000mg. The trouble was mostly with evening meal. Post prandial was around 9-13 even with two metformin. Evening meal is always 45-60g of carbs. Made appointment with GP who advised “those numbers are not bad.” In fairness to GP he then said he will speak to D nurse who deals with cases on daily basis. He spoke with her and asked me to go and have a chat with her. I had a chart showing blood readings and the carbohydrates consumed with every meal. Nurse agreed with my increase to 4 metformin per day.
5.5 last night before meal. 13.10 1 hour after and 8.5 2 hours after. Two metformin taken with meal. Carbs in meal = 60
The nurse knows I am very strict with what I eat and closely monitor my blood sugar levels. She was impressed with the detailed chart I gave her and suggested an appointment at Nutrition and Dietetic Clinic which I will attend next week and she would send my detailed chart to them in the post.
Surely this appointment can’t be to receive advice on what I should eat, quantities etc. as I have a strictly controlled diet, 130g carbs per day, and I ensure I receive healthy levels of protein, fat, minerals and vitamins etc.
Current BMI is 22.
Any thoughts?

Hiya, sorry about your setback. A|t a guess, too many carbs! Maybe not immediately after you stopped the Newcastle diet, but time and your diabetes have moved on. I couldn't safely manage 60 carbs at one meal.
Just a thought. Others' views to follow no doubt.
 
Al, that would be way too many carbs for me; I have to stick at as little as possible while still eating! Hopefully I am going to try the ND in a couple of weeks to see if it kick starts my insulin secretion/sensitivity with the hope that I can have a few carbs again.
 
60 carbs per meal is way too much for me as well. Try less carbs to get to a max 2.0 rise after meals.
 
I agree with the above, too many carbs. If I eat around 20g carbs at one sitting my levels go sky high, so I stick to around 30g a day, maybe 50g on a bad day.
 
Agree with all 60g per meal is absolutely massive amount.
I take about 30 to as much as 60 in a day but 60 in a day is my absolute top figure
On a good day I could have only had 7 to 10g
Also metformin will take time to start working.
Did you do full 8 weeks of newcastle diet?
 
Thanks for the replies. I calculated the carbs exactly; closer to 49g carbs. My maximum total carbs per day has been 130 for some time now. Must re-visit the whole thing again and re-jig. Have been vegetarian for more than two years so meat and fish no longer an option for my low carb menu. Thanks again, everyone.

Hi @al_leister I would struggle too on so many carbs. I manage on around 40g a day without meat and fish. The metformin doesn't do much for bringing down numbers, but has other benefits so is worth taking.

What's your a1c and or bg levels right now like before and after meal

Breakfast, morning tea break and lunch BS levels are fine. Breakfast = 14g carb, morning tea and lunch = 22g carb.
Waking BS = 5.4 - 6.1. After morning tea and lunch 2hr postprandial is always below 7. Before evening meal BS is 5.1 - 5.8. This more than doubles after eating evening meal and takes approx 3 hrs to settle to 7 or below. Will simply cut evening meals in half or more in terms of carbs.
I will reduce evening meal to 15g carb which makes the daily total = 73g carb.
Just picked up a good vegetarian low carb book so will be busy with new recipes.

We all vary tremendously with our carb tolerances, and maybe yours has just dropped?
Any changes in exercise level? Sleep patterns? Any possibility of hidden infections? Raised stress levels? (Ok, I am stretching now!)

My carb tolerance is at its best when I spread the carbs through the day, but my lowest tolerance is first thing in the morning. So I tend to have a protein/fat brekkie then spread the carbs evenly after that.

Thanks, Brunneria. Very helpful! For sure, mycarb tolerances have dropped. Like overnight. Previously could eat 50g carb for evening meal with 1 metformin 500mg and be down to 5.7 - 6.2 after two hours. Even with 2 metformin 500mg two hour reading is 8 - 10. My last A1c was 5.8 though that will increas now if I don't re-jig the carbs. The sudden, almost overnight change was like being diagnosed again for the first time. I will still attend the Nutrition and Dietetic clinic appointment tomorrow; it will be interesting, I'm sure. Thanks again for the info.

I am so ******* angry!

A half an hour of my life; I will never get back.

Dietician appointment today.

So why have you come to see us? They asked.

I didn’t request the appointment, I said. The practice nurse made the appointment when I explained my sugar levels had risen sharply and suddenly and I presented her with a chart showing detailed information on my diet and daily carb intake.

I will not go into the fine details as you all know them so well so just a little bit below.

Straight off they told me type 2 diabetics shouldnt be testing their blood and a man should be eating 250-300g of carbs per day.

I couldn’t sit like a dumb animal so I challenged them. There was a student dietician in the room as well.

I offered to eat to their recommendations for 1 week and return with blood sugar levels and show them how out of control the levels are.

No, we don’t want you to do that, they said.

Why not? I said.

We can only tell you what the official advice is, they said.

They didn’t even have the whit to say, if you find on our diet that your sugar levels are crazy then go to your GP for stronger/different medication.

So I have simply taken time off work to attending a “telling off” regarding my daily carb intake.

I left saying, I am sorry this has been a waste of your time and mine.

I'm so sorry. And I feel your frustration.

Nowadays, I say 'I control my carbs.' or 'I control my portion sizes.'
and they seem quite happy with that. Which just shows how narrow their thinking is!

But since I got the Trudy Deakin 'Eat Fat' book, I took it to my latest endocrinology appointment, in case they started driveling the usual NHS high carb mantra. Didn't need it for that appt, but I will have it with me for the next.

I've got one of these coming up this summer so I'm not handing out advice to do the nodding dog thing until I've done it myself. Until then ... bummer, it's not right, it's unprofessional, it's negative, it doesn't pay any heed to the patient as an individual and it happens time and again. Hope you feel a bit better for your very reasonable protest.
 
Forgot to mention. whey i told them my daily carb intake was 130g they looked at each other as if i was going to die; at their feet, there and then.
