A Very Mysterious Case of Glucose Dysregulation

Hi, I am sorry for such a tone, I was not aware this happens on those forums and that you had to deal with similar situation in the past. I tend to overreact and am actually getting help for this from my doctors. I had once measured a hypo which was at 3.9 mmol/l and yes it was not pleasant. I will try to get back to my doctor to describe those issues and get an opinion. Much appreciated.

Every glucose monitor have a false rate of 0.5 mmol, meaning your reading of 3.9 mmol could be anything from 3.7-4.2 mmol. Your reading could as well have been 4.0-4.2 mmol which is considered normal, also I’ve heard of several cases of non diabetics getting 3.9 mmol, but usually never lower.

With that said I would not consider 3.9 mmol a hypo, it is a whole lot of difference between lets say having 2 mmol and 4 mmol.

I still think your symptoms are not blood sugar related. I feel completely fine at 4.0 mmol, it is at 2-3 mmol I start to feel bad, even thought I know this can vary from person to person.
 
Anxiety causes sweating and a faster heart beat. However, when I ate too low carb I had your symptoms. Horrendous palpatations.
Your swinging bgs sound food induced. Why have you decided to break your keto diet?

I hoped that my failure to pass GTT was because of low carb. I was told by an endo to resume carbs and I did for 2 weeks which is plenty of time to get back to glycolysis instead of ketosis. It did not help. So I was back to low-carb and cannot get back to that doc because of many reasons, I'd need referral from GP but they cannot send me anywhere with normal HbA1c. They seemed concerned about the failure to adapt again to glucose but they just told me to keep eating what I am eating and this way control the BGs. I am suspicious of my low c-peptide and those occasions of glucose spikes at least once a week drives me insane. It is not much of a spike but it kills my mood, causes tremendous brain fog and feeling of being ill. I am already on medications for anxiety and everything is fine until those spikes happen.

I now sit with a headache whole day, feeling weak and peeing more than usual but my BG is below 6.0 I haven't eaten for 4 hours and it is 5.6 or something like that.

Overall, I am frustrated by the causes because it seems to me that those episodes are getting more frequent and worry me.

Can you please answer some question? Would make it easier to understand your condition.

1) Did you take a new OGTT after you had stopped your keto diet?

2) And how did you know that getting back into glycolysis didn’t help?

3) You say you get a spike each week, but what do you mean with spike? What blood sugar value do you have then? How high does it go (when you take a reading at home).

Also c peptid can change and go up or down based on diet and how much insulin your body need. So living keto and on low carb will give a temporary lower c-peptid. Whats abnormal and of a concern would be a low c-peptid reading when you eat and consume much carbs.

1) No, the OGTT was administered a week after the previous one, on the same conditions. I do not consider it valid as my endo would otherwise diagnose diabetes if he thought differently. A few doctors still claimed that non-diabetic would maybe show impaired glucose tolerance on low-carb but not diabetes. Hence, doubts.

2) I measured my blood glucose which consistently got above diabetic range after eating carbohydrates for 2 weeks continously without values lowering or showing any progress returning to normal, where normal is below 7.8 mmol/l after 2 hours. It was more like 10-11.

3) Well, anything above 6.0 mmol/l makes me feel weird, sweaty and irritable with a headache and moody/tired. It is definitely not a spike to most humans but it makes me consistently have the previously mentioned symptoms. I always see values above 6 with them and never feel such issues when below.

Unfortunately, the endo did not test the insulin production under the carb load, I do not doubt he saw no need for that, but it still pesters at the back of my mind because of my failure to get 2) under control.

4) 8-11 times a day. Every second night I find waking up with a need to go to the bathroom.
 
I hope you won't take this the wrong way, but you have referred to your anxiety over having normal blood glucose levels and feeling worse when you are slightly up or down. This is the anxiety speaking, you are second guessing what is happening, you are worried over how your body reacts to certain foods which you may be intolerant to. You have symptoms similar to a hyper or a hypo because even though your glucometer says you are in normal range, you feel that you are not. You actually feel worse if you go below four even though that is not a hypo. You are questioning your ability to cope if you have high or low blood glucose levels. You are worried about your anxiety issues. You are repeating the advice given and don't except the advice, you think it is wrong. You are certain that things are not right, despite the contrary.
You want to come out of ketosis, despite feeling better doing it. It's driving you mad!

I know these symptoms, feelings and actions, my anxiety crippled my brain and its not nice.
It is the anxiety you need to treat and trust the advice given.
See your GP, tell your GP, what you are feeling. Why you want to find out why you are feeling this way.
It's nothing to do with blood glucose levels.
Your anxiety has you worried about being ill!

2) Do you mean your blood sugar are 10-11 two hours after eating? Or did you only get 10-11 mmol shortly after eating?

If not, what is the highest value you ever measured at home?

3) No, mmol over 6.0 won’t give you these problems. Here I have to agree with @Lamont D that this has more to do with your anxiety, causing you to think or feel that carbs makes you sick when you go over 6 mmol.

so, You are low carb, what does a typical days food consist off??

Beef, chicken, broccoli, cauliflower, eggs, bacon, cheese, radish, various nuts, sunflower seeds, etc. I make meals of all those ingredients.

Having 7-8-11.1 mmol after 2 hour can indicate impaired glucose tolerance, but it can’t still be a result of keto diet pushing your blood sugar higher than normally because your body hasn’t been used to high glycemic food again.

What is the highest reading you’ve ever got at home? And how many readings have you had similar to that?

My highest was around 11.9. My highest when on low carb was yesterday 6.8 mmol/l after walking which used to reduce my BGs, now they increase instead for some reason. I had consistent readings in 10-11 range while atttemptin reintroduction of carbs. I have consistent readings of mostly between 5-6 when eating low carb diet.
