Dapagliflozin, do’s and dont’s

I’m new here so apologies if I don’t follow protocols.
Male.69yrs.Type 2 Metformin, slow release 2000mg a day.
Been tracking my blood pressure at request of doc, for 7 days then review 4th June.
Lost 3 stone from 1/1/26 to 31/3/26 calories based 1500 a day.No alcohol, still doing it holding weight at 15 stone 7.Small amounts of wine weekly now about 6 per week.
In last couple of weeks although my blood pressure seems stable, I’ve been feeling, vacant, woozy, not with it, losing concentration and eye focus, what’s going on I ask myself, migraines as well weekly, but only visual zig zags and speech disturbance, previously every 3 or 4 months.
Take 10mg in morning of Dapagliflozin which I know has similar side effects.
My recent Diabetes review showed in May26 HBa1c has dropped from 81mmol to 51mmol.
My Accucheck blood sugar pin prick test shows between 7.8 and 8.8mmol/L most Fridays weekly , fasting.
Any ideas what’s causing what I assume to be low blood sugar? When I eat a meal, carbs and protein it goes in 20mins?
My Metformin has just been reduced by Doc last couple of days to 1000mg, she thinks now maybe over medicating.
Any observations welcome or questions, been type 2 for 12 years or so, this has never happened before and I’m a bit anxious.
 
Hi @Tonyk777 and welcome to the forum. Well done on reducing your weight and HbA1c. How about testing your blood sugar when you’ve “been feeling, vacant, woozy, not with it, losing concentration and eye focus,” to see if it’s actually low blood sugar, if it isn’t low and it’s fixed by eating, maybe it’s just because you are hungry because of your low calorie intake?

I've got to agree with @Rachox, it's not an assumption you can really make without doing a pin prick test.

(And congrats on your massive improvements.)
 
Acknowledged.While having the problem yesterday test showed 8.4mmol/L on Accucheck device so that doesn’t appear to be low sugar.It’s not low blood pressure either.

I’ve just looked at your Type 2 journey and all the HBa1c measurements over many years, and you clearly reversed the issue.Well done indeed, your fortitude is inspiring.
I don’t know Tirzepatide but guess you were injecting?
One of the Doctors I occasionally see at my Practice now the Senior Partner says you can’t reverse Diabetes, and is quite adamant about it.
Dexcom one I think you wear on your arm? But is it not associated more with Type 1? Can you get those measurements through an app?
You clearly are an experienced operator with Diabetes
 
I think the 'reverse' issue is a grammatical one. Yes we can manage diabetes, we can, by diet and lifestyle changes, even have non-diabetic blood readings over a long period, but if we go off the straight and narrow, we will get diabetic symptoms again, and thereby diabetes. If we think 'reverse' means we can live like non-diabetics without getting symptoms, well, no we can't. We will always have diabetes.

Thank you for your lovely comments. Tirzepatide is Mounjaro which I use a small dose of for my diabetes. As @Outlier says reversing diabetes, in my view too is just grammatical. I would prefer to label my profile as excellent control but reversing was the only option close to expressing my status.
Dexcom ONE + is a continuous glucose monitor which yes I wear on my arm. I can’t get it on the NHS but have a 90 day subscription which reduces the cost. I can see my levels through an app on my phone and also can see them on my Apple watch. Also I can see them on my car screen with Apple CarPlay which is really cool!
I put a lot of my success down to the help I have received over the years from this forum

Yes, I googled Tirzepatide and saw it was Mounjaro. Think I might get a Dexcom ONE +.
Not sure how accurate or worthwhile my weekly Accucheck tests really are.Have a good, albeit pretty hot day !
