Basal rates

Can anyone help me?
I am on the insight pump and need to adjust my basal rates.
If for example my blood sugar is high at 10am should I change my 8am setting, or do you change the 10am?

You need to change the basal rate ahead of when you want to see the change take effect. However, while the "2 hours before" value is always thrown out there, keep in mind that it is only yet another "rule of thumb". These always are a best fit for the fictional average person.

We all need to start someplace when tweaking a basal, but what really matters is what works best for your experience of D in your body. Quite possibly that means changing 2 hours ahead. But maybe it means 3 hours, or 1 or 1 1/2, or ...
 
I have always changed my rates 1 hour before bus as mentioned you need to try it out and tweak if necessary.....

I wouldn't be changing anything just from a high reading at one particular time though, you would need to be observing the blood sugar level over an hour or two to see what happening in that time window....

I suspect you know this and are just asking about when to make the change.......:)
 
You should check your blood sugar levels 4 hours a head in this case and see how they are then and only alter in very small amounts A wee tweak is always better than a big one.
 
How are you testing/checking your basal rates, @lisacp? Are you using a CGM (Continuous Glucose Monitor) or just periodically checking your BG with a meter? I think how complete a picture you have of a trend affects your choice as to "when" you decide the basal might need to be tweaked.

You'd also want to have confidence that the upward/downward trend in your basal which you're considering making a change for is a pattern and not just something you've seen once. Although I might make a (small) change for a downward trend based on less evidence than an upward one. I don't like going drifting high, but I consider lows the bigger PITA. :eek:

One other thought I've been having lately is to not change my basal based on trends which happen towards the "end of life" of my pump's infusion set. Perhaps my basal is wrong, but perhaps it may also just be poor absorption because my infusion set has gone wonky on me.
