Recently experiencing lows immediately after evening meals and spikes a several hours later. It started with meals such as spag Bol and roast dinners. I appreciate fatty foods can delay carbs release but I’m not convinced this is the cause. It appears my Novorapid works extra quick in the evening (yet not any other time of day)!? I used to inject 20mins before evening dinner and now I resort to injecting straight before. My lows after dinner can last a couple hours so I need to eat just to keep level over ‘4’ but it catches up with me several hours later when I’m asleep. I would really like to resolve the issue without the need to double dose before sleep (always dangerous) or until I feel the rise in middle of night to correct. sometimes I feel it may just be one of those things that Type 1’s experience due to my body changing. I’ve achieved excellent control for over 32yrs & I split dose my tresiba basal which works perfectly. Any suggestions welcome

Have you tried decreasing your evening novorapid?

Now oddly, I was wondering what time the split dosage of basal was administered & about the action of that before the novorapid is actually kicking in? If @LWA is treating a hypo whilst dinner is metabolised that may be what is "catching up??"
More carbs than the bolus was anticipating due to what was thrown at the hypo..?

Yes I do reduce my evening novorapid to suit but it ultimately means I’m then double dosing later which is what I am trying to avoid. Bizzarely this never happened in my initial 30 yrs of being type 1 but does now . I inject my basal at 8am and 9pm (2 hours after my evening meal) so I’m fairly sure this doesn’t contribute to the issue ‍♂️

You say you are tresiba - why do you split this - Tresbia is 36 hours lasting so not meant to split as it doesn't have the same release like Lantus for exmaple which may only last 18 hours. Have you just changed to Tresiba? However i can locate a graph of how Lantus works but can't fine one for Tresiba, so i know you have dismissed your basal but it could be part of the issue.

it might be worth going back to the beginning and doing some basal testing to make sure your tresiba doses are correct ....here is the best website link i have found for explaining how to basal test https://www.mysugr.com/en/blog/basal-rate-testing/

Thanks for the link - been on tresiba for 3 years and it’s proved to be a fantastic upgrade from my previous (levimir) many people suggest it works better for some people when split - when I injected 24 units in one go it gave me severe lows after 8 hours. I have tested my background insulin which proves to remain very flat for any 24hr period
