Help needed!!

Hi seeking some advice if I may..? How is it possible that my blood sugar level can go from 2.2 up to 21.8 in the space of 2 hours?? Only food I had in that time was a small glass of orange juice and two bits of toast?!? I am still on novomix 30 and take 2 injections a day. The hospital records show that I had switched to the 4 a day regime in 2012, but clearly not, as all my prescriptions have been for novomix??? Any help at all? Over the last month or two, I have had 2 hypos requiring an ambulance and hospital visit, as well as over 25 readings of blood sugar in excess of 20...? Please help me with any advice?!?! Thanks

Hi @ThePQ88 , welcome to the forums.

Can I ask you to be a bit more precise with regards to the quantities involved?. OJ may have a lot of sugar in it, some people use it to avert hypo's. 2 'bits' of toast doesn't really tell us much.

A slice of bread is typically in the region of 15g of carbohydrate, if you had 2 slices - that's +30g and the OJ. How much insulin did you inject for this?

Do you carb count? Give us a bit more info and we might be able to help you.

I use Humalog 25Mix and have been on it for just over 2 years now. I seem to get on OK generally speaking but my carb intake is only around 120g per day at the moment and I do go high sometimes.

Hi - I take around 40 units in the morning (before breakfast) - and around 34 in the evening (before dinner).....whilst I was in the hospital, every nurse couldn't believe that I was still on the 2 a day regime. When I finally got through to the right department, they told me that their records said that I had switched to the 4 a day regime in 2012!? How is that possible?

Are you seeing the hospital doctor and getting meds from your GP? My initial thought is that your GP has failed to action an instruction from the specialist ????

I started on Novomix 30 and didn't get on at all well with it. I find the Humalog Mix25 much better so maybe talk to your doc about a different insulin (or a basal /bolus regime).

40 units of Novomix 30 would give you around 12 units of rapid acting insulin and 28 units of long acting. Do you know what your carb/insulin ratio is?

The rapid acting part is pretty much spent within 4 to 5 hours so if you had your toast and OJ at midday or later then you will not have enough rapid acting insulin in your system to cope with it.

What is the timing of your injections with regard to the food you've mentioned?

I did my reading at around 8.30am this morning and it read 8.9. I then did my usual shot, had a bowl of special K and got on with my day. I then tested at about 2pm and found it to be 2.2. I have an appointment with the hospital on Tuesday - hopefully I'll have some more answers then

If you didn't eat a meal around midday along with whatever activities you did then this would be why your BG was 2.2

If your injecting twice a day, you need to inject a good 30 mins before your meal (less if your pre-meal BG is low), you need to eat again 5-6 hours later and not miss out meals, you may also need a snack in between meals too.

On top of this you need to test your BG's before each meal, and at least 2 hours after each meal.

If your BG drops low then your body will produce its own glucose to help it rise and you will get a high BG reading a few hours afterwards, if you over treat a hypo or hyper then you can end up bouncing between highs and lows for a day or so.

I have had my appointment at the hospital today and they have switched my regime to 4x a day - I start it tonight! I hope this will stop the above happening to often again!! Thanks for comments so far guys

Good luck - test lots and lots, even during the night if you need to.
