High reading of 2hr ogtt

Since I’ve found that I have pre-diabetes. I start low carb diet, cut all sugar in these three months. The fasting range: 5.5-6.2 post meal: 6.6-8.0 almost.

Today I do my OGTT which are requested by my doctor. 70 mins later I prick finger that the blood sugar up to 18, i wondering if I didn’t following the rule: before ogtt. Ensure 150g carbs intake daily. The low carb diet will make my insulin are not sensitive to sugar so that the glucose got so high? or I get worst of diabetes?

Your valued information is so important for me. Thank you.

How did you do the OGTT? Were you fasting at the time? (overnight, no food etc).
What sort of glucose did you drink?
Did you test immediately before the drink?
Have you finger pricked tested at 120 minutes? (2 hours)
How many finger tests have you done and what were the results?

Thanks for your detail remind:

I was fasting to do the 75g pure glucose OGTT,
The 18 reading was pricked at 70 minutes by finger test, but not tested 120mins. because the nurse will take my arm blood at 120 minutes, but one hour was so high. I think 2hour reading won’t get far from it.

Before, I usually take 2 times finger tests daily
Fasting+ 2hr post meal, test everyday and now near 3months, keeping fasting <6.2 and post meal <8. never seen that high reading at my usual test. But this OGTT totally knock me down.

I just test now, it drop to 5mmol . but already 8 hrs apart from OGTT . just eat some nuts only during this time

what is your thought on my situation?

I don't have any thoughts because it is the 2 hour level that matters. You need to wait for that result.

75g of glucose is a huge hit of sugar, so you can expect a high reading after an hour. It is how long it takes to come down that matters.

For someone doing low carb an off the cuff OGTT will almost certainly produce bad results. There is a specific pre-carb process to be followed, due to the differences between a bodily system used to carbs and one that has been fat adapted. It is necessary to effectively carb load (around 150 grams of carbs) for around 3 days prior to an OGTT if you are fat adapted.

Thank you for the information. I will talk with my GP regarding this matter of low carb.

This is my record today

75g glucose is the norm for OGTTs,
The norm for pre test diet for a two hours tolerance test is 150g of carbs.

The high reading in the first hour is due to insulin resistance and low initial insulin response, which produces the high spikes.

What was the 2 hour result?
Without that we don't really know what to think

Today i got my result from the last OGTT test.
Fasting 6.6 and 2hr was 20.9.

GP is shocked about that high and ask me prick finger immediately one more time. ( I have had some vegetable juice at 8am and pricked at 1230pm) , the reading is 5.5

He still not put me on medicine, because he said my last HBA1C and fasting are not so seriously. But he send me to the specialist and make appointment at the end of this month. Doing further test.

My last HBA1C was 7.4 in Feb. Not start to Low carb at that time.

I remember that one hour was 18 why two hour didn’t drop but rose.

Do i really got a severe diabetes?

Goodness, that 2 hour reading was very high. It was a good idea to send you to a specialist. Hopefully he can do more tests and sort out some appropriate medication.
