Ketones low, blood sugar high

Hi.

I'm just over a week into a keto diet after my T2D diagnosis. Oddly despite eating strictly, my ketones this morning and this afternoon have been around 0.2 - 0.3 mmol/l. I would have expected and hoped them to be within the optimal range of 0.5 - 3.0, which they were a few days ago.

My blood sugar is high - 10.3 mmol/l. Might that be having an effect on my ketone production?

If blood glucose is high then in all likelihood you will have elevated circulating insulin which in turn will severely limit or entirely block ketogenesis. You can’t really burn stored fat in the presence of insulin. Give it more time. Glucose levels don’t always plummet straight away. In fact in some circumstances you may even see rises as the liver deflates. Meantime try to lean toward extra dietary fat to give the process a helping hand.

A week is a pretty short amount of time .. I know ketogenic diets can have a fast effect but I'd start to think months and not days.
If you are producing ketones on a blood ketone monitor than you are in effect "in ketosis" whatever the level but as @Jim Lahey says its still very early days.
I'm guessing you have just started BG measurement so you won't know how high your sugars were beforehand?
Hopefully you will see more drops and lower levels over time.

Yes I've only just started recording my blood glucose. The nurse actually told me not to bother as he was more concerned with my ketosis making me unwell.

I'm actually feeling pretty horrendous at the moment. I feel like I'm getting what might be a false hypo every afternoon. Right now I have a headache, and feel confused. Sometimes I feel weak and a bit confused. I'm hoping this is normal for a new diabetic just starting to lower their blood sugars!

Yeah be careful dropping levels too fast. I’ve only ever had one false hypo but let me tell you there was nothing false about it. Never felt worse, before or since. Genuinely feared for my life.

Maybe try testing to see what levels you are at when you feel off.. make sure to have enough salt too that can be a bit low if you have changed your diet fast. I add a pinch to hot drinks.
