Monitoring booklets

Anyone have a booklet for recording daily bloods with a space to also imput carbs at meals

Animas do a pump journal which has small boxes for bolus, basal, carbs etc.

I don't know if you can get your hands on them but the DAFNE diaries have space to record carbs and insulin doses, also meters such as the Contour USB meter let you input the carbs with your insulin doses.

Yes, the DAFNE ones are very good and I think you can get replacements from the DAFNE website although there may be a charge. Whenever I have a 6monthly check-up at the hospital though there are always plenty of freebies at the clinic.

I have a home made spreadsheet with all that and more for my pumping information.......

I could email you it...........?

You can just print it out and fill it in on a day to day basis..........

I have used a home made copy of John Walsh's chart, printing it out as needed. This works ok if you just want a paper version.

(Then I used a one touch meter that you could input data on).
Now I've got a newer meter that doesn't have all bells and whistles of the One touch one so I'm going to go back to sidiary

I have contacted dafne and bought a booklet
Have no printer and so needed proper diary as am to go to dns shortly who likes ALL the detail in front of her.HMMM will ask her for one from her dept
