Moving Tresiba Dose Time

Morning lovelies.

As some of you are no doubt aware, I’ve been whining a lot recently about having to wake up at 3-6am to soma correction dose for the raging dawn phenomenon I’ve been getting, and having to eat when I don’t want to in order to avoid a slump late afternoon/early evening.

I currently take my Tresiba at 8am, so am thinking of moving it to 8-10pm. What’s the best way of moving my time? Just slide it back through the day a few hours at a time? Or miss tomorrow morning’s dose and just take it tomorrow night instead? Obviously I don’t want to stack doses...

Any advice gratefully accepted. I can’t remember how I went about splitting Lantus a million years ago.
 
I don't know but be careful, the consultant I see said it's 3 days before dose adjustment (not timing) shows and settles, 5 days for some people, my 'guess' with the >36 hours life would be missing the morning and having it at night, but there'll be a high sugar at some point...

Thanks Kev, I’ve found it takes around 5 days to see a dose change. Tbh, I don’t think I like Tresiba. I thought that the really long nature of it would solve a lot of my issues, but in fact it’s created more problems. It’s just so inflexible.

If the issues you have with it can be helped by moving it forward 12 hours you may see an improvement, but that's 'may' and you'll have a bad sugar week trying it out, problem there tho is if it creates new issues at new times :/ for me getting a more consistent and flatter baseline is the key, I can do flexible to suit my 'regime' if needed and if it does the same thing each day (unlike Lantus as that can seem to please itself) I'd work at it,

Which is easy to say whilst still on Lantus :p
 
Hi @Mel dCP personally I would just do the switch, I wouldn't be worried so much as it's not a dose adjustment and as its still in your system then it shouldn't make much of a difference to change times, I was told when I was on it that it didn't matter if you missed your dose as you could take it when you remembered and having only forgot to take it once then taking it later on it didn't make any difference when I did.

Well the issue I’m currently having is that I go high at around 4-6am (if I don’t wake up to correct with Novorapid) and low around 4-6pm, so I reckon if I just shift it by 12 hours, it might help.

I’ve already taken this morning’s shot at 8, so you reckon I could just miss it tomorrow and take it at 8pm instead? Or stagger it by taking tomorrow’s at 2pm and then Monday’s at 8pm?
 
Hi @Mel dCP its entirely up to you if you want to stagger it but it shouldn’t make much difference either way, just keep doing a test to check it for your own peace of mind if you need to :)

You mean scan every ten minutes instead of every twenty? :D I read the instructions and it said to make sure you don’t take two doses within 8 hours, so I could take a dose tonight, I suppose.
 
Tbh, i changed to night time and it made no differance to my night time highs, the only thing was it was difficult remembering to have it ! Ive gone back to 8am over 3 days ....no differance to BGL .
 
Sorry just a test every hour if your feeling nervous however I doubt it will make much difference, also I am inclined to agree with Ann as i’m not sure this will make a difference to your overnight highs, I would do a 3am test for a few nights to observe though.
 
I'd probably just take a half dose am then full dose pm. Any consequent highs during the day can just be patched with fast acting but as changes take a while to impact you'll likely not notice the missing half for a few hours. As basal is slow, there's unlikely to be any major dramas with stacking it.

When your MiaoMiao arrives, it's obviously good for things like hypo alerts, but an often overlooked aspect is hyper alerts. I've got mine set to a relatively low 7.6 so I can pin any night rises with 1 or 2u and avoid waking to >10.

Obviously the main aim is to sort the DP with basal tweaking but the hyper alerts will at least let you be a lot more precise in the meantime by just waking you when there's a definite rise.

PS: I'm going to have to get me a MiaoMiao now - all you youngsters enthusing about it is making me and my blucon feel like an old duffer with a Betamax!

As a kid I was frequently mocked if mates watched a video at my parents as we had Betamax :p

We did too. And a BBC computer. My dad had a knack for picking tech about to go obsolete. Kiss of death!

And thank you for all your advice, I’ve decided to take tomorrow’s dose at two pm and the day after at 8pm, which is the time I’m aiming for. Just means the next two days doses will have to last for 30 hours, so I reckon that’ll be ok. I’ll monitor carefully :)

Indeed! There's a colleague in my office with a T1 husband and we chat about T1 matters every now and then. I had no idea till she mentioned it that miaomiao was also a nickname for an arrestable drug. That's what's happened since I've turned 50!

I had no idea either, until my 15 year old mentioned it... I’m too old for this stuff!
