This used to happen to me a lot with big restaurant meals, and I was also on Humalog and Lantus (clue that we are both type 1). I think it was a mix of factors. Overestimating the carbs, extra fat in the meal to delay the blood sugar rise, delays in the food arriving at the table, and these were the days when I would inject 15 min before. All those factors compounded and made hypos shortly after starting a restaurant meal a common occurrence.
I now wait until I have eyes on the food before I inject, and I guess I'm just a little more cautious in the quantity I inject. That seems to have done the trick. But maybe it's just my salty sea dog luck.
