Hello, we are a group of students at GA London researching possible solutions to help someone diagnosed with prediabetes to make necessary lifestyle changes to avoid the disease progressing.

We would need 2 minutes of your time to answer 5 simple questions. Form is completely anonymous and no personal data is collected.

Sorry but I for one am not happy to press random links to forms provided by people posting for the first time to any website including this one. If you pose your questions here you may find that people are prepared to comment, though I would expect that you can glean many of the answers to your questions from a review of the actual topics on here.
 
I did it for what it was worth, I do not think my answers will help being a T2 and not prediabetic.

We need more people to respond, so please participate, it is only 1 minute (5 questions).
 
I have responded, but the questions just assume we stray from our plan, so quite irrelevant for me. You may be better adding more questions relating to diet and exercise, and what the plan actually is.

Completed but your survey doesn't really cover what you say you are covering...If you want to poll prediabetics then would it not be better to exclude Type 1 and 2's in the survey? Also the assumption that people cannot stick to their plan is a bit odd for the newly diagnosed as they probably didn't have/weren't given a plan to start with. Not sure what your survey is trying to uncover but I would be wary of any findings from it in its current form.
