Hi @saffy I can't help you much because I'm T1 but I've had many years of not so good bgs in the past and I wanted to say that you shouldn't blame yourself for not having the best bg control. There are lots of reasons why many people struggle with their diabetes and victim blaming helps no one.

As regards the insulin, is your reason against it the weight gain? As a T1 who has struggled with weight gain for many years, I find that going relatively low carb has helped me lose weight and though I'm not quite where I want to be I'm near enough. But I've always prioritised my blood sugars over my weight.

And while 91 is high mine has been higher in the past so please don't give up. There are a lot of meds for T2s out there other than insulin so maybe you should talk to your gp about other options?

Good luck and virtual hugs.
