To Medicate or not to Medicate

This is going to be a bit graphic about bowels so if you're squeamish probably better to stop reading now! As some of you will know from other posts I had some minor surgery last week (an endimetrial ablation). Because I have OSA (Obstructive Sleep Apnoea) they decided to give me a spinal without sedation and I was absolutely fine with the procedure (even got to see my insides on camera!). I was in overnight and home the next day with minimal pain and everything seemed fine. Planned to go back to work on Monday 11th all being well. In fact from the surgery point of view the results have been a dream!

After surgery it can take a wee while for your bowels to move but everything seemed fairly normal by Wed evening but by Thursday evening I was experiencing some major evacuation of the food I'd eaten. I don't have a sore stomach and have felt normal hunger etc, but an hour after eating everything just transits right on through.... :***:

I went back to work on Tuesday and lasted two hours before having to be taken home (an hour of which I'd spent in the toilet) and saw the doctor who said it could be either a co-incidence and I've picked up an infection or it could be that the nerves in my digestive system have been affected by the spinal and need a wee while to recover. From my perspective I am leaning towards the latter because my blood sugars have stayed excellent (and usually spike if there is any kind of bug or infection) and also the output is looking more and more like food that just hasn't really been digested mixed with a whole lot of water. He didn't want to prescribe Immodium or anything similar because if it's a 'bug' then it's best to be flushed out my system. I put in a sample but they've told me it will be a WEEK before they get results - which is of no use to man nor beast quite frankly!

I'm very conscious of preventing dehydration now and getting a bit worried about my levels of salts etc. I have lost 13lbs in weight in just 8 days. Now in fairness the surgery has resulted in coming off a high dose three times a day of the hormone Norethisterone which can affect both your BG and can cause you to gain weight (though I have lost 44lbs while on it by sheer determined effort) - so there might be an element of my body reacting to that change with the weight loss, but even taking that into consideration it's WAY too much.

Up until yesterday I was still trying to eat relatively normally despite the 'throughput' and managing around about 1200 calories a day on LCHF and continuing to take my meds. I'm on 4 x 500mg Metforming and 1 x sitagliptin/januvia plus one Perondopril in the morning (well before food). My BG this morning was 5.1 which is about normal since I've gone LCHF but today I haven't been able to face any food at all - just water. In order for the Metformin not to upset my stomach I have to take them with food - so now I'm in a quandry. Sitagliptin is one of the drugs they don't recommend you take if you're vomitting or have loose bowels so I'm not so worried about that - and not so worried about my bloods as they are so good.

So do I eat something and take the metformin or can I just skip it and if I DO just skip it for a few days am I then asking for trouble going through the whole 'my stomach doesn't like this' when I reintroduce the drug later?

My instinct would be to stop the Metformin and then re-introduce it, one a week, then 2 a week, etc, when I was feeling very much better and the "through-put" had stopped. But do eat something un-upsetting, eg eggs, soup, fish - and keep drinking.

However, I am not qualified to suggest anything! except phone the surgery and talk to your GP.

I hope you get better soon - not a nice situation to be in!

Hi. Can I ask the obvious question; are you on the Slow Release (SR) version of Metformin? If not then ask for this as it should help with the stomach problems.

Daibell,

I'm not on SR, but to be honest as long as I take it with food I now don't have many problems with Metformin when my digestive system is functioning normally. I did at first though when introducing it - had to stick with just one tablet for almost 6 weeks before I could progress to two and so on. Really don't want to go back to that, but then again I was hoping, given my bloods over the last few months, to be coming off at least some of the metformin all together shortly (if not all).

I was put on to the meds after my blood sugars went on a steep rise following an assault nearly three years ago (HBA1C went up to 10.1 / 86). It took a long time to recover in terms of stress levels particularly but I'm most definitely there. My last HBA1C was 7.6 / 60 in August. Having switched to LCHF I've been fasting testing in the mornings at 5-6 since the start of this year (confirmed against the hospital measurements last week) with periodic pre and post meal testing showing much the same, even dropping into the 4s on occasion. I've been shoving so many tablets down my gob for the last few years (10 a day then adding on the supplements and vitamins.....! :***: ) that I'm fairly desperate to start to cut down on unnecessary meds wherever I can.

This, however, was not the way I planned to do it! I'm going to have to go back to the docs tomorrow anyway as I'm still experiencing 'through put' even with just being on water and I can't wait for a week for some results to come through to have it treated in some way. Apart from anything else I really need to get back to work!

It is important to keep hydrated when you have this type of problem, I always drink a couple of Dioralyte drinks available from all chemists whenever I suffer an upset tummy and or diarrhea. They will help replace the electrolytes and salts you are loosing errrr, through the other end!

Ask your pharmacist about it and do mention your recent procedure just to be on the safe side. And speak to your pharmacist or better still your doctor before stopping any other meds
