Too high especially at night

It sounds like her blood sugar was rising pretty fast - hence the 18 half an hour after you corrected at 16.

Did you give extra insulin to allow for the fact the BS was over 12?

Could it be a delayed rise from the evening meal?
 
What was the last time of food given tonight what was test 2 hrs after that food?
What was the food?

Would be good to see a full 24 hr history to get a full picture ie if high all day or any low's.

If leveks are above 12.0 it is normal to need more insulin to get them lower than the standard corrections. I double the recommended correction bolus if above 12.0 but you need to find the correct level of increase for your daughter....
 
I'm going to suggest you download and use the mysugr app if you can, it helped me massively with spotting patterns and getting good advice :)

Sounds to me like her lantus dose isn't enough and her novorapid meal doses are too high. How are you working out how much rapid to give? Do you have a diabetes nurse you can call for dose advice between consultant appointments?
 
When calculating the bolus dose, do you use a fixed ratio like 1 unit per 10gm carb? After a lot of experimenting I have settled on 1:15 at breakfast, 1:12 at lunch and 1:9 at supper. I suspect the different levels are down to taking the basal dose in the morning. Its effect tapers off over the 24 hours despite what they claim.
