Type 2, LADA or Type 1 advice

Hi there,
I’ve been diabetic since 2005 when age 42. Following a horrible burglary attack when I was living in South Africa, I was in deep shock and was all over mentally and physically. I was diagnosed a few months later with my BG through the roof, infections and I could not stay awake in the day and was drinking a mass through the night. Seemingly my autoimmune had kicked in as a result of the trauma and that was the explanation for my diabetes onset. I was immediately put on Metformin.

Prior to this, I was very fit and health and a regular runner and walker. I continued the running when I moved to the UK, but was on insulin within 6 or 9 months as the metformin was insufficient to control the BG. For many years I lived with daily swings of Lows (sub 3.5) to highs (20+).

Last year, I was referred to a DSN as part of the GP’s annual review. This was brilliant, for the first time in 15 years, I worked with someone whom really understood and helped get me on track. She suggested using the Freestyle Libre (self funded) and I’m now under great control - HBA1c ~ 6 from ~10.1. The DSN discharged me because of my good control, which disappointed me, but I understand she has very limited capacity.

Under the DSN, I also started balancing my insulin to Carbs and she helped my tune this. As part of this I started a low carb diet, with the BG Insights from the Libre and the balance of insulin. I’ve lost about 15 kg and feel so much better.

I’m now taking between
- 0 and 8 units of NovoRapid with meals depending on Carbs
- 20 units morning and night of Levemir background.

But something was nagging in my mind from my original diagnosis re the autoimmune in South Africa and then being treated as Type 2 when in the UK. So I asked the Diabetic Nurse at the GP if I could be tested to see what type of Diabetic I am.

With so much insight in my BG tracking form the Libre, I see that sometimes by BG is flat, but floating at 6 to 10 but will not come down over time on its own unless I take 2 units of NovoRapid but without eating. This brings my BG nicely in range. But should I need to take Insulin even when not eating ? If I’m type 2 should the BG come down slowly when not eating ?

Ive now had the ANTI-GAD AB test in January. It came back as 7 u/ml which is a Low POSITIVE. I understand this is a test for Type 1 diabetes.

The Diabetic Nurse said if it came back positive, then I would need to change medication, but there has been no follow up on this yet.

I have asked for a follow up call and this may be in the next week.

Sorry for the long post, but I’m wondering if anybody has similar experience in their condition and thoughts if I’m Type 1, 2 or LADA. ?

Positive GAD = T1 (LADA is type1) If you're already on basal and bolus insulin and it's working well you already have the preferred treatment for T1.

Hi. I've been down a similar route except in my case I believe the cause was virus. If you need insulin to control your BS and you are not overweight then being T1 is likely and the GAD even if low would tend to confirm that. A c-Peptide test might show your level of insulin production but I would just assume you are T1 and you therefore have the right treatment. Anyone with excess weight and on insulin might actually be an insulin resistant T2 in which case weight loss might obviate the need for insulin with just tablets needed.
 
The GAD test returned 7, which I understand is a low positive.

It took some months to get an appointment, but the consultant feels that I am Type 2. When I was diagnosed in 2005, I was very fit and healthy, but more recently I put on weight. Now recently (in the last 6-9 month) i have lost 20kg. This is since I have started with the Freestyle Libre and changed to accurate Carb Counting and following LCHF.


Looking back on my appt with the consultant, I think he misunderstood that I had lost 20kg after being diagnosed rather than being very fit when diagnosed, but having the horrible burglary attack in South Africa. i.e. the cause of my autoimmune.


Now the consultant says I need to C-Peptide test to confirm the Diabetes type. Given it took 5 months to get the GAD and follow up appt, I have decided to go private for C-Peptide rather than to wait.

BTW - My health insurer says they will cover the test but not consultant costs.

Can anybody advise:
1) where I can get the C-Peptide done
2) if they will do it for me personally or only as a consultant referral ?
3) if they will provide the results to me so I dont have to wait longer for another consultant appt to get the results ?

LADA takes years to fully develop, you make some insulin until you don't. A lot of us have been misdiagnosed for years, I was for 9 years. Luckily I was put on insulin fairly early because medications made me sick. The GAD antibodies slowly decrease over time, but a c-peptide will tell you if you are making insulin and how much.

I have no idea about the UK, in the US you can just pay for the blood test by way of a mail order company.
 
@Bwel Medichecks do a C-peptide year for £99, but they often have a sale. I would imagine BUPA and Nuffield hospitals would also do it, especially if you want a consultants appt too. Give them a call I’ve always found them very helpful.
 
I just went to the Dr and said I think I may be diabetic, they told me to go to a website (can't remember unfortunately) to book it at the local hospital and a form to take so you t was done on NHS.

Turns out I did the right thing.
C-peptide, very small amounts of production.
GAD about 120 (fairly low but higher than the norm)

T1 confirmed
